多层螺旋CT低剂量扫描诊断肺结节的临床可行性研究 被引量:1

摘要 目的分析研究多层螺旋CT低剂量扫描诊断肺结节的临床可行性。方法选取我院2012年5月—2016年5月诊治的肺结节的患者48例作为研究对象,对其实施多层螺旋CT低剂量扫描;并以同期收治的48例接受常规剂量扫描的肺结节患者作为对照。对不同扫描剂量下的结节检出情况进行对比研究,记录其辐射剂量相关参数。结果经多层螺旋CT常规剂量和低剂量扫描的患者,分别扫描到肺结节57个和55个,结节平均直径分别为(11.6±5.7)mm和(11.5±5.9)mm,组间对比,差异不具有统计学意义(P>0.05);其毛刺征、分叶征、钙化征、空泡征、支气管征、胸膜粘连征以及密度不均匀灶等形态学特征的检出率比较,差异均不具有统计学意义(P>0.05),具有较高的一致性。但低剂量扫描组的DLP、total MAS、Eff MAS等辐射剂量指标均低于常规剂量扫描组,组间对比,差异具有统计学意义(P<0.05)。结论多层螺旋CT低剂量扫描的诊断效果和安全性俱佳。 Objective To study the clinical feasibility of low-dose spiral CT in the diagnosis of pulmonary nodules. Methods 48 cases of pulmonary nodules treated in our hospital from May 2012 to May 2016 were selected as the subjects, To apply multislice spiral CT low-dose scanning, compared 48 patients who received routine dose scanning for pulmonary nodules. The detection of nodules under different scanning doses was compared, and the radiation dose related parameters were recorded. Results Patients with multi-slice spiral CT, conventional and low-dose scans were scanned for 57 and 55 lung nodules respectively. The mean diameter of the nodules was (11.6±5.7) mm and (11.5±5.9) mm respectively, compared with those of the patients with multiple doses of spiral CT and low dose scanning, the difference was not statistically significant (P〉0.05). There was no significant difference in the detection rate of morphological features such as burr sign, lobulation sign, calcification sign, vacuole sign, bronchus sign, pleural adhesion sign and density unevenness, the difference was not statistically significant (P〉0.05), with a high consistency. But the radiation dose index of DLP, total MAS, Eft MAS in the low dose scan group were lower than the conventional dose scanning group, the difference between the two groups was statistically significant (P〈0.05). Conclusion The diagnostic effect and safety of multi-slice spiral CT low dose scan are excellent.
