摘要
目的探究冲吸式口腔干预对住院ICU患者呼吸机相关肺部感染的影响,为临床治疗提供参考依据。方法选择2015年3月-2016年3月医院ICU住院患者88例,将患者随机分为对照组和干预组,各44例,对照组给予患者常规棉球擦拭进行口腔处理,干预组给予患者冲吸式口腔干预,比较两组患者临床指标。结果经治疗,两组患者第三天起干预组的评分显著高于对照组;干预组的白细胞计数低于对照组,降钙素原显著低于对照组;干预组的PaO_2高于对照组,PaCO_2低于对照组;干预组IL-6、IL-8、IL-10、TNF-α水平低于对照组;干预组的肺部感染评分显著高于对照组,肺功能指标优于对照组,两组比较,差异均有统计学意义(P<0.05)。结论冲吸式口腔干预能够有效降低呼吸机相关肺部感染的发生率,提高住院患者的生活质量。
OBJECTIVE To investigate the effect of aspiration oral intervention on ventilator-associated pulmonary infections in hospitalized ICU patients,so as to provide references for clinical treatment.METHODS A total of 88 cases of hospitalized ICU patients from Mar.2015 to Mar.2016 were selected,which were randomly divided into control group and intervention group,with 44 patients in each group.Patients in control group were given oral treatment of conventional cotton ball wipe,and in intervention group were given aspiration oral intervention.The clinical indexes were compared between the two groups of patients.RESULTS After treatment,the score of intervention group was significantly higher than that of control group(P〈0.05).The white blood cell count and C-reactive protein of intervention group were significantly lower than those of control group(P〈0.05).The level of PaO2 in intervention group was significantly higher than that in control group(P〈0.05),and the level of PaCO2 in intervention group was significantly lower than that in control group(P〈0.05).The levels of IL-6,IL-8,IL-10 and TNF-αof intervention group were lower than those of control group,The pulmonary infection score of intervention group was significantly higher than that of control group,the pulmonary function index of intervention group was significantly better than that of control group(P〈0.05).CONCLUSION Aspiration oral intervention can reduce the risk of ventilator-related pulmonary infection in hospitalized ICU patients,and improve the quality of life of inpatients.
