摘要
医疗服务利用行为是个人健康状态的直接体现,但其亦可能受到社会经济地位、居住地等因素的制约。本文借鉴健康生态学理论分析医疗服务利用行为,将个体比作处于生态系统的生物体,把个体生活的工作、家庭等社会环境看做一定范围内的"人类生态系统",介于家庭和工作微系统对个人的直接影响,本文对老年人的工作及家庭环境重新梳理,着重探究家庭微系统中代际支持、工作微系统中社会资本与医疗服务利用的联系,并使用CHARLS 2013数据进行实证分析,结果显示:(1)代际支持对医疗服务的使用具有显著正向影响,(2)社会经济地位对老年人医疗服务利用存在"断点"特性,(3)医疗服务费用影响因素的性别差异凸显。
Medical service utilization is is the direct embodiment of the individual health status,and but it may also be restricted by the social and economic status,residence and other factors. This study based on health ecology theory to analyze the medical service utilization behavior,the individuals are compared to organisms in the ecological system,work,family and social environment is considered as a range of ' human ecology system' to explore the impact of family microsystem system and work microsystem on the individual behavior of the utilization of medical care. Considering that the elderly family and work environment changes with age,this paper reviews the elderly family and work environment and focused on the relationship between intergenerational support,social capital and medical service utilization. Cross-section data of CHARLS( China Health and Retirement Longitudinal Study) 2013 is adopted to conduct the empirical study. The results showed that:( 1) Intergenerational support has a significant positive influence on the use of medical services;( 2) the effect of income conditions on medical service utilization shows the characteristics of ' break point';( 3) gender differences of influencing factors on medical service cost was significant.
