摘要
目的探讨舒适护理在降低尿道下裂患儿Snodgrass术后尿瘘发生率中的临床效果。方法将2010年1月~2016年12月在医院行Snodgrass术的200例尿道下裂患儿按照随机数字法平均分为两组,100例患者采用常规护理(对照组),100例在对照组基础上给予舒适护理(观察组)。将两组患者哭闹情况、首次排便时间、大便性状情况、术后切口并发症及尿瘘发生情况进行比较。结果观察组患者哭闹次数显著少于对照组,且哭闹时间显著短于对照组(P<0.05)。观察组患者不同原因引起疼痛次数显著少于对照组(P<0.05)。观察组患者首次排便时间显著短于对照组,且大便性状湿软例数显著高于对照组(P<0.05)。观察组患者术后切口并发症及尿瘘发生情况显著低于对照组(P<0.05)。结论在行Snodgrass术的尿道下裂患儿护理中选择舒适护理可显著降低患者疼痛感,减少术后伤口并发症及尿瘘发生情况,促进患者康复,值得在临床上推广使用。
Objective To explore efficacy comfortable nursing on decreasing the incidence ol urinary hstuta in cnllclren with hypospadias after Snodgrass surgery.Methods 200 cases in the hospital with hypospadias undergoing Snodgrass surgery from January 2010 to December 2016 were selected and divided into two groups according to the random number method.100 cases were given routine nursing care （the control group）,100 cases were given comfortable care on the ba- sis of the control group （the observation group）.The number of crying,postoperative pain,tbe first defecation time,stool con sistency,the incidence of postoperative urinary fistula and incision complications were compared between two groups.Results The number of crying patients in the observation group was significantly less than that in the control group,and the crying time was significantly shorter than that of the control group （P〈0.05）.The number of pain caused by different causes in the observation group was significantly less than that in the control group （P〈0.05）.The first defecation time of the patients in the observation group was significantly shorter than that of the control group,and the number of stool soft wet symptoms was significantly higher than that of the control group （P〈0.05）.The incidence of postoperative urinary fistula and incision complications were significantly lower than thoset of the control group （P〈0.05）.Conclusion Nursing care of children with hypospadias undergoing Snodgrass surgery in the choice of comfort care can significantly reduce the pain of patients,reduce the incidence of incision complications and urinary fistula after operation,promote the rehabilitation of patients.h is worthy of promotion in clinical use.
