T_2Flair高信号血管征与颅脑CTP-TTP的关系初步研究 被引量:5

摘要 目的研究急性大脑中动脉栓塞中T_2Flair高信号血管征与颅脑灌注时间相关各参数相关性。方法回顾性分析我院就诊的46例急性大脑中动脉栓塞患者,均进行MR T_2Flair及CT颅脑灌注检查,测量双侧大脑中动脉供血区颅脑灌注TTP及MTT参数并计算其差值(ΔTTP、ΔMTT),通过修改的ASPECTS(modified Alberta Stroke Program Early Computerized Tomography Score),对T_2Flair高血管征进行评分,Spearman相关对所得颅脑灌注参数及T_2Flair评分结果进行分析。结果 38例检测到T_2Flair高信号血管征(平均值4,范围0~7)。与对侧正常大脑半球相比,当ΔTTP小于1s时,检测不到T_2Flair高信号血管征。T_2Flair高信号血管征与ΔTTP(r=0.748,P<0.001)呈显著相关、ΔMTT(r=0.338,P=0.021)呈中等程度相关。结论在大血管闭塞的情况下,ΔTTP与T_2Flair高信号血管征有很好的相关性,T_2Flair高信号血管征可能依赖于软脑膜侧支循环逆行血流灌注时间。 Objective To investigate the relationship between hyperintense vessels (HVs) on fluid-attenuated inversion recovery (FLAIR) and Time to Peak (TTP) on Cerebral CT Perfusion (CTP).Methods We retrospectively reviewed 46 patients with proximal middle cerebral territorial infarction who underwent MR Imaging and CT perfusion, and calculated the difference in TTP(ΔTTP) and mean transit time (ΔMTT) between infarcted and normal hemispheres.We scored HVs on FLAIR imaging using a modified Alberta Stroke Program Early Computerized Tomography Score (ASPECTS).Spearman correlation analysis was used to explore the relationship among the FHV score and CTP parameters.Results We detected HVs on FLAIR images in 38 patients (median score, 4;range, 0 to 7).Comparison of infarcted and normal hemispheres demonstrated absent HVs on FLAIR imaging when the ΔTTP was too short (〈one second).HVs scores on T2-FLAIR showed a significant positive correlation with ΔTTP (r=0.748,P〈0.001), while ΔMTT showed moderate significant relationship (r=0.338,P=0.021).Conclusion In cases of large arterial occlusion, ΔTTP and T2Flair HVs have good correlation,the hyperintensity of vessels on FLAIR images may be dependent on retrograde filling time of the leptomeningeal collateral circulation.
