摘要
目的观察美洲大蠊干燥虫体提取物对面部中重度痤疮光动力治疗一次后局部不良反应消退的促进作用。方法将66例中重度痤疮患者随机分为观察组和对照组各33例。观察组光动力治疗痤疮1次后创福康胶原贴敷料及美洲大蠊干燥虫体提取物冷湿敷面部(美洲大蠊干燥虫体提取物3次/d,10min/次;创福康1次/晚,15mim/次),对照组外敷创福康胶原贴敷料(1次/晚,15mim/次)。1周及2周复诊观察,观察两组不良反应的发生、消退情况以及对治疗后患者生活质量进行评分比较。结果观察组不良反应消退总有效率为77.78%,明显高于对照组的总有效率62.22%,差异有统计学意义(P<0.05),观察组生活质量评分高于对照组,差异有统计学意义(P<0.05),观察组较对照组不良反应消退速度快,患者生活质量评分高,提高光动力治疗的安全性,对后续光动力治疗及患者的依从性有很大意义。结论对于中重痤疮患者光动力治疗后,局部应用美洲大蠊干燥虫体提取物能促进不良反应的消退速度,更高效改善患者的生活质量。值得临床推广美洲大蠊干燥虫体提取物应用于中重度痤疮光动力治疗后的修复治疗。
Objective To observe the promoting effect of the extract of Periplaneta americana on the local adverse reac- tion after the treatment of severe acne. Methods 66 patients with moderate to severe acne were randomly divided into observation group and control group, each with 33 cases. In observation group, the wet and cold facial compression of a Chuangfukang collagen dressing and Periplaneta americana dry body extract were applied after Photodynamic therapy for acne 1 times （ dry extract of Periplaneta americana larvae 3 times / day, 10 minutes / time; Chuangfukang 1 time/night, 15 minutes / time）. The control group were treated with a Chuangfukang collagen dressing（ 1 time/night, 15 minutes/time）. 1 week and 2 weeks follow-up observation of adverse reactions, the occurrence of extinction and the quality of life in the two groups after treatment were compared by scores. Results The total effective rate was 77.78% in the observation group, which was significantly higher than that in the control group 62. 22% o the difference was statistically significant（ P 〈 0. 05 ）. The quality of life score of the observation group was higher than that of the control group, the difference was statistically significant （P 〈 0. 05 ）. Compared with the control group, the adverse reaction rate and the quality of life score of the observation group was higher. It was of great significance to improve the safety an com- pliance of photodynamic therapy. Conclusion For the treatment of moderate and severe acne patients after photodynamic therapy, topical application of Periplaneta americana extract can promote the elimination rate of adverse reactions, and improve the quality of life of patients. It is worth to promote the clinical application of Periplaneta americana extract in the treatment of moderate to severe acne after photodynamic therapy.
