血清白细胞介素-6、触珠蛋白及淀粉样蛋白A对慢性阻塞性肺疾病患者肺部感染的鉴别诊断研究 被引量:25

Value of serum interleukin-6,haptoglobin,and amyloid A in differential diagnosis of pulmonary infections in patients with chronic obstructive pulmonary disease

摘要 目的探究慢性阻塞性肺疾病并发肺部感染患者血清白细胞介素-6(IL-6)、触珠蛋白(HPT)、淀粉样蛋白A(SAA)水平,为慢性阻塞性肺疾病并发肺部感染提供诊断依据。方法选取2014年1月-2016年1月医院接受治疗的354例慢性阻塞性肺疾病患者为研究对象,其中并发肺部感染的92例患者为A组,262例未发生肺部感染的患者为B组,选取354名健康体检者为C组;对A组患者进行菌落培养并鉴定肺部感染病原菌种类;采用酶联免疫吸附试验对三组研究者血清白细胞介素-6进行测定并加以比较;使用双光径免疫浊度分析仪检测三组研究者触珠蛋白以及淀粉样蛋白A水平;将A组患者根据是否患有呼吸衰竭分为慢性阻塞性肺疾病非加重患者和加重患者,比较两组患者以上指标含量水平;运用ROC曲线测定血清白细胞介素-6、触珠蛋白以及淀粉样蛋白A对慢性阻塞性肺疾病并发肺部感染的诊断价值。结果 A组的92例患者共检出病原菌164株,其中革兰阳性菌36株占22.0%、革兰阴性菌107株占65.2%、真菌14株占8.6%、其他菌种7株占4.3%;A组患者血清IL-6含量为114.42±17.82ng/L、HPT含量为1.88±0.36g/L、SAA含量为271.05±22.45g/L,B组患者血清IL-6含量为83.46±15.59ng/L、HPT含量为1.56±0.24g/L、SAA含量为225.49±20.68g/L,C组研究者IL-6含量为31.03±19.71ng/L、HPT含量为0.89±0.21g/L、SAA含量为48.24±19.37g/L,三组数据比较差异具有统计学意义(P<0.05);并发肺部感染的患者中,加重患者血清IL-6、HPT以及SAA水平均高于非加重患者。结论血清白细胞介素-6、触珠蛋白以及淀粉样蛋白A均可对慢性阻塞性肺疾病并发肺部感染进行预测,临床可加以控制。 OBJECTIVE To explore the levels of serum interleukin-6(IL-6),haptoglobin(HPT),and amyloid A(SAA)of chronic obstructive pulmonary disease(COPD)patients complicated with pulmonary infections so as to provide guidance for diagnosis of COPD complicated with pulmonary infections.METHODS A total of 354 COPD patients who were treated in the hospital from Jan 2014 to Jan 2016 were recruited as the study objects,92 patients who were complicated with pulmonary infections were assigned as the group A,and 262 patients who did not have pulmonary infections were assigned as the group B;354healthy people who received physical examination were set as the group C.The bacterial colonies were cultured for the patients in the group A,and the pathogens causing pulmonary infections were identified.The serum IL-6level was determined by means of enzyme-linked immunosorbent assay and was compared among the three groups of study objects,the levels of HPT and SAA of the three groups of study objects were detected by using dual-path immuno-turbidimetric analyzer.The patients in the group A were divided into the exacerbated COPD patients and the non-exacerbated COPD patients according to whether they had respiratory failure.The levels of indexes were compared between the above two groups.The value of serum IL-6,HPT,and SAA in diagnosis of COPD complicated with pulmonary infections was determined with the use of ROC curve.RESULTS Totally 164 strains of pathogens were isolated from 92 patients in the group A,including 36(22.0%)strains of gram-positive bacteria,107(65.2%)strains of gram-negative bacteria,14(8.6%)strains of fungi,and 7(4.3%)strains of other species.The serum IL-6level was 114.42±17.82ng/L in the group A,83.46±15.59ng/L in the group B,31.03±19.71ng/L in the group C;the HPT level was 1.88±0.36g/L in the group A,1.56±0.24g/L in the group B,31.03±19.71ng/L in the group C;the SAA level was271.05±22.45g/L in the group A,225.49±20.68g/L in the group B,48.24±19.37g/L in the group C,and there was
