摘要
目的探讨髋关节置换后血栓性事件与血小板改变之间的相关性。方法选取2013年4月至2015年4月骨科行人工髋关节置换术的88例患者作为此次的研究对象,其中髋关节置换后发生血栓性事件的37例患者作为观察组,术后未发生血栓性事件的51例患者作为对照组。对两组患者单双侧手术情况、性别、年龄、体重以及术后血压、血小板含量进行统计比较,并进行多因素logistics回归分析;比较两组患者手术前、手术后1 d、手术后5 d血小板含量的变化情况。结果观察组患者双髋节置换16例(43.2%)、单髋节置换21例(56.8%);对照组患者双髋节置换11例(21.6%)、单髋节置换40例(78.4%),差异有显著性(P<0.05)。多因素logistics回归分析显示男性、高龄、体肥以及血小板含量是髋关节置换后血栓性事件的危险因素。观察组术血小板含量较对照组处于较高水平,差异具有统计学意义(P<0.05)。结论髋关节置换后血栓性事件的发生与血小板具有相关性,在血小板升高时发生率相对较高。
Objective To explore the relationship of thrombotic events and changes in platelet after hip replacement.Methods 88 cases with artificial hip replacement in our hospital from April 2013 to April 2015 were selected, in which 37 patients with thrombotic events after hip replacement were treated as observation group, 51 patients with no postoperative thrombotic events were treated as the control group.Operation type （single or bilateral operation）, gender, age, body weight and blood pressure, platelet content were compared.And multiple logistic regression analysis were performed.The changes of platelet content of 1D and 5D after operation in two groups were compared before and after operation.Results In observation group, there were 16 cases （43.2%） with double hip joint replacement, 21 cases with single hip joint replacement （56.8%）.In control group, there were 11 cases with bilateral hip joint replacement （21.6%）, 40 cases with single hip joint replacement （78.4%）, there was significant difference （P〈0.05）.Multiple logistic regression analysis showed that male, age, body fat and platelet content were risk factors for thromboembolic events after hip replacement.In the observation group, the platelet content was higher than that of the control group （P〈0.05）.Conclusion After hip replacement, the evaluation of platelet is associated with the incidence of thrombotic events.
