阴道分泌物念珠菌感染三种检测方法比较分析 被引量:9

摘要 目的探讨三种检测方法对阴道分泌物真菌检测结果的差异。方法收集镜检阴道分泌物真菌阳性和阴性标本各200例,用PCR技术鉴定其念珠菌类型,再随机选择荧光PCR核酸检测结果阳性标本30例进行真菌微生物培养,验证分型结果的正确率。结果常规镜检阳性200例,荧光PCR核酸检测鉴定阳性189例,其中白色念珠菌157例(78.5%),光滑假丝酵母菌24例(12%),热带假丝酵母菌6例(3%),白念合并光滑酵母菌2例(1%),其他真菌11例(5.5%)。常规镜检阴性200例,荧光PCR核酸检测鉴定阳性58例,其中白色念珠菌50例,光滑假丝酵母菌6例,热带假丝酵母菌2例;随机选择荧光PCR核酸检测结果阳性标本30例真菌培养结果吻合率97%。结论门诊常规检验中,真菌涂片镜检应为首选。对于门诊镜检阴性而又有临床症状的患者,建议进行分子生物学基因鉴定。镜检和分子生物学方法可以相互补充,但应以分子生物学方法为标准。 Objective To compare the differences in detection of Candida infection in vaginal secretion by using 3 types of methods. Methods We collected 200 Candida positive and 200 negative vaginal secretion specimens from routine microscopic exami- nation, respectively. The Candida types were identified with PCR technique. Then, we randomly selected 30 PCR positive samples for fungal microorganisms' cuhure to demonstrate the accuracy of classification results. Results Of the 200 microscopic positive samples, 189 samples were identified as positive by using fluorescence PCR method. In these of fluorescence PCR positive samples, 157 were Candida albicans (78.5%) ,24 were Smooth candida yeast ( 12% ) ,6 were Candida tropicalis (3%) and 2 samples were combined Candida albicans and Smooth candida yeast. There were 11 cases with other fungus (5.5 % ). Of the 200 microscopy negative samples,58 samples were identified as positive in which 50 were Candida albicans,6 were Smooth candida yeast and 2 were Candida tropicalis. In the randomly selection of 30 fluorescent PCR positive samples, the coincidence rate with fungal culture results was 97%. Conclusion In the routine clinic' examination, microscopic examination of fungal pictures should be the first choice. At the same time, for outpa- tient with microscopy negative but has clinical symptoms ,it is recommended to do molecular biology gene identification. Microscopic ex- amination and molecular biolozical methods can be complementary but molecular method should be used as standard.
