摘要 目的通过分析中性粒细胞/淋巴细胞比率(NLR)与肺栓塞患者的临床转归,探讨NLR对肺栓塞患者预后评估的价值。方法将135例肺栓塞患者分为发生临床不良事件组(A组)及未发生临床不良事件组(B组),对两组患者的基线资料、血气分析、血细胞分析、肾功、血脂、心肌酶、心肌损伤标记物、心脏超声等部分临床资料进行统计学分析。结果 A组WBC、N、NLR、cTNT-hs、Pro-BNP与B组差异有统计学意义(P<0.05);Pro-BNP、c TNT-hs、CK-MB的ROC曲线下面积位于0.50-0.70之间,有较低准确性,WBC、N、NLR曲线下面积分别为0.78、0.81、0.87,有一定准确性,NLR最优,最佳临界值为4.87。结论 NLR对肺栓塞患者的预后判断有一定的价值,可能作为评估肺栓塞患者病情进展的常规检测指标。 Objective To investigate the prognostic value of patients with pulmonary embolism (PE) by neutrophIL-to-lymphocyte ratio (NLR) and their clinical outcome.Methods A total of 135 patients with PE enrolled in the study, and they were divided into two groups according to clinical adverse event occurred.Their clinical data, blood gas analysis, blood count, renal function, blood lipids, myocardial enzyme, myocardial injury markers and cardiac ultrasound were recorded for statistical analysis.Results The differences of WBC, N, NLR, cTNT-hs and Pro-BNP showed statistical significance between the two groups (P〈0.05).The area under ROC curve of Pro-BNP, cTNT-hs and CK-MB was 0.50-0.70, and the area of WBC, N, NLR curves were 0.78, 0.81 and 0.87, respectively.The accuracy of NLR was the best and the best cut off value was 4.87.Conclusion NLR has a certain value in predicting the prognosis of patients with pulmonary embolism and may be used as a routine index to evaluate the progression of pulmonary embolism.
