摘要
目的了解抗精神病药物在老年精神病患者中的应用情况以指导老年人合理用药。方法选取我院于2015年4月至2016年4月期间，包括慢性精神科一科、慢性精神二科两科室在内的200例老年精神病患者作为调查对象，判断分析患者病情并调查询问其药物使用类型、用药日剂量、不良反应等药物使用情况。结果老年精神病患者以精神分裂症为主（60．5％），与其他类型疾病相比具有显著性差异（P〈0．01），其后依次为阿尔茨海默症、脑血管所致的精神障碍、抑郁症等疾病，抗精神病药物在患者中的使用率为90．5％，另有15例患者使用两种以上抗精神病药物，口服抗精神病药物中氯氮平、利培酮、盐酸氯丙嗪等抗精神失常药占主导地位，分别为29．5％、24．0％和13．5％，患者所选用的抗精神病药物的日服用剂量均低于平均口服量，但差异无统计学意义（P〉0．05）。结论老年精神病患者多选用单一的常规抗精神病药物进行治疗，其日服用剂量均略低于平均值。医生应根据老年精神病患者的用药特点结合药物特性，加强合理、有效、安全的用药理念在老年患者中的普及与宣传，并通过定期调查分析其药物使用情况，以真正建立合理用药体系。
Objective To investigate the clinical application of antipsychotics in elderly psychiatric patients in the elderly in order to guide the rational use of drugs. Methods in our hospital from April 2015 to April 2016 period, including chronic psychiatric department, 200 cases of eld- erly patients with chronic mental illness, the spirit of the two families two departments as the research object, judgment analysis of the disease and the type of inquiry on drug use, dosage, usage of drug adverse reactions. Results in elderly psychiatric patients with schizophrenia （60. 5%）, as compared with other types of disease with a significant difference （P〈0.01）, followed by Alzheimer＇s disease, mental disorders caused by cerebrovascular disease, depression and other diseases, the use of antipsychotics in patients with the rate of 90.5 %, another 15 cases with more than two kinds of antipsychotic drugs, oral antipsychotic clozapine, risperidone, Antipsychotic drugs such as chlorpromazine hydro- chloride accounted for the dominant position, respectively, 29. 5 %, 24. 0 % and 13.5 %, patients with the use of antipsychotic drugs, dosage were lower than the average dose, but the difference was not statistically significant （P〉0.05）. Conclusion elderly psychiatric patients choose single conventional antipsychotie drugs treatment, the daily dose was slightly lower than average. The doctor should be according to the charac- teristics of the elderly patients with bxsychiatric medication combined with medicinal properties, strengthen the reasonable, safe and effective med- icine concepts in elderly patients with popularization and the propaganda and through regular surveys to analysis the drug use, in order to truly establish system of rational drug use.
