摘要
目的 探讨腹腔镜子宫楔形切除术联合曼月乐治疗对子宫腺肌病患者临床疗效及生活质量的影响.方法 将106例子宫腺肌病患者采用随机数字表法分为两组,各53例.两组均行腹腔镜子宫楔形切除术治疗,观察组在此基础上观察组联合曼月乐治疗.于手术前及手术后6个月采用视觉模拟评分法及月经失血图比较两组痛经程度及月经量,比较血清糖类抗原125水平及生活质量综合评定问卷评分,统计两组术后12个月疾病复发率. 结果 手术后6个月,观察组痛经程度、月经量及血清糖类抗原125水平均显著低于对照组（P〈0.01）,生活质量综合评定问卷评分显著高于对照组（P〈0.01）.观察组手术后12个月复发率为0,对照组为7.5%,观察组显著低于对照组（P〈0.05）.结论 腹腔镜子宫楔形切除术联合曼月乐治疗子宫腺肌病可有效改善患者的临床症状,提高生活质量,降低疾病复发率.
Objective To explore the effects of laparoscope-assisted cuneihysterectomy （LAC） plus levonorgestrel intrauterine system （Mirena） on efficacy and quality of life of adenomyosis patients.Methods A total of 106 adenomyosis patients were divided into to groups of 53 ones each according to random number table.Both groups received laparoscope-assisted cuneihysterectomy and observation group was plus Mirena.Dysmenorrhea degrees and menstrual blood volume （MBV） with the Visual Analogue Scale （VAS）and Pictorial Blood Loss Assessment Chart （PBAC）as well as serum carbohydrate antigen 125 （CA125） levels and Generic Quality of Life Inventory-74 （GQOLI-74） score were compared between two groups before operation and in month 6 after operation, relapse rates added up in month 12 after operation.Results In month 6 after operation dysmenorrhea degrees, MBVs and CA125 levels were significantly lower （P〈0.01） and GQOLI-74 scores higher （P〈0.01） in observation than in control group.Relapse rate was 0 in observation and 7.5% in control group and the former significantly lower than the latter （P〈0.05）.Conclusion LAC plus Mirena could effectively improve patients＇ clinical symptoms, boost quality of life, and lower relapse rate.
