摘要
目的回顾性分析消痔灵注射内痔术后所致并发症及不良反应病例,明确其并发症及不良反应发生机制,提出消痔灵注射用药规范化建议。方法搜集2012年1月至2016年12月四川大学华西医院痔瘘中心收治的因内痔注射消痔灵后,发生较为严重并发症及药物不良反应病例共27例,分析其并发症及不良反应的发生机制。结果 27例住院患者中,下消化道出血有10例,占37.04%(10/27);肛门疼痛有7例,占25.93%(7/27);直肠肛管狭窄有4例,占14.81%(4/27);过敏反应有3例,占11.11%(3/27);肛周脓肿有2例,占7.41%(2/27);直肠阴道瘘有1例,占3.70%(1/27)。不良反应的发生机制与消痔灵成分、消痔灵注射配伍、浓度、注射剂量、注射深浅、注射部位及患者的病情和个体差异等有关。结论注射消痔灵时,严格遵守适应症,仔细评估患者情况,遵照规范化操作,才能降低并发症及不良反应的发生率。
Objective To review and analyze the cases with complications and adverse reactions after hemorrhoids surgery with Xiaozhiling injection,investigate the mechanisms and put forward the advices on the appropriate medication.Methods The data of 27 cases with serious complications and adverse reactions after hemorrhoids surgery with Xiaozhiling injection from January of 2012 to December of 2016 in West China Hospital of Sichuan University were collected to analyze the mechanisms of those complications and adverse reactions.Results Among the 27 cases,there were 10 cases with hmatochezia （37.04%）,7 cases with anal pain （25.93%）,4 cases with anorectal stenosis （14.81%）,3 cases with hypersensitivity reactions （11.11%）,2 cases with perianal abscess （7.41%）,and 1 case with rectovaginal fistula （3.7%）.The mechanisms of the adverse reactions were related with the components of Xiaozhiling,combination with other drugs,concentration,dose,inj ection depth and location,and the conditions and individual differences of patients.Conclusion The incidence of complications and adverse reactions can be reduced by following the indications of surgery strictly,assessing patients′conditions before surgery and performing surgeries according to the standardized procedures.
