摘要
目的探讨基层医院腹腔镜结直肠癌手术的临床疗效。方法选择2013年1月至2014年12月腹腔镜结直肠癌根治术患者50例及开腹结直肠癌根治术患者43例，比较其手术时间、术中出血量、术中淋巴结清扫数目、术后胃肠功能恢复时间、术后并发症发生率、住院时间及近期复发率。结果两组年龄（P=0．657）、性别（P=0．359）、体重指数（BMI）（P=0．707）、肿瘤部位（P=0．992）、肿瘤分期（P=0．358）差异无统计学意义。腹腔镜组手术时间（193．70±39．54）ml，比开腹组（155．30±23．72）min长，腹腔镜组术中出血量（112．26±39．58）ml，比开腹组（170．30±108．39）ml少，差异有统计学意义（P〈0．001）。腹腔镜组术中淋巴结清扫数目（22．30±6．22）枚，开腹组术中淋巴结清扫数日（23．56±6．88）枚，两组差异无统计学意义（P=0．356）。腹腔镜组术后胃肠功能恢复时间及术后住院时间分别为（58．65±16．65）h、（10．65±6．20）d，比开腹组（98．63±29．78）h、（15．32±6．54）d明显缩短，差异有统计学意义（P〈0．001）。术后并发症发生率（P=0．914）及术后近期复发率（P=0．238）差异无统计学意义。结论腹腔镜结直肠癌可以达到根治效果，且术中创伤较小，恢复快，值得基层医院推广应用。
Objective To investigate clinical significance of laparoscopic colorectal cancer surgery in primary hospital. Methods 50 cases of laparoscopic radical resection and 43 cases of open radical resection for colorectal cancer were included to evaluate their operation time, blood loss, lymph node dissection number, postoperative gastrointestinal function recovery time, postoperative complication rate, hospital day and recurrence rate. Results There was no significant difference between the two groups in age （ P=0.657 ） , sex composition （ P=0.359 ） , body mass index （ P=0.707 ） , tumor location （ P=0.992 ） and stage （ P=0.358 ） . The operation time of laparoscopic group was （ 193.70 ± 39.54 ） min, longer than laparotomy group （ 155.30 ± 23.72 ） min. The amount of bleeding during laparoscopic surgery was （ 170.30± 108.39 ） ml, less than open group （ 112.26 ± 39.58 ） min. The number of lymph node dissection in laparoscopic group was （ 22.30 ＋_ 6.22 ） , there was no difference compared with the open group （ 23.56 ＋ 6.88 ） . The recovery time of gastrointestinal function and the length of stay in the laparoscopic group were （ 58.65±16.65 ） h and （ 10.65 ± 6.20 ） d, compared with the open group （ 98.63 ± 29.78 ） h and （ 15.32± 6.54 ） d was significantly shorter. No significant difference was found in two groups of postoperative complications （ P=0.914 ） and recurrence （ P=0.238 ） . Conclusion Laparoscopic colorectal cancer can achieve the curative effect, and the surgery is less invasive, quick recovery, it is worth to promote the application in primary hospital.
