摘要
目的:比较容积MRI与^1H-MRS对颞叶癫痫的诊断价值,以期指导癫痫病人选择有价值的确诊方法。方法:经临床及EEG证实为颞叶癫痫的病人50例。应用3.0T场强的磁共振设备分别对双侧海马结构进行容积MRI、^1H-MRS采集。分析两种方法各自的诊断符合率,然后进行卡方检验。另用3.0T磁共振设备对43例正常志愿者进行容积MRI、^1H-MRS采集,作为对照组将所得结果与本文参考文献中的正常参考值进行t检验及方差分析。结果:计算出容积MRI和^1H-MRS各自的诊断符合率,经卡方检验后P〈0.05,两种方法的诊断符合率统计学上有显著性差异;本组正常对照的NAA/(Cho+Cr)值与参考文献的NAA/(Cho+Cr)值经方差分析、t检验,对照组中值统计学上无显著性差异(P〉0.05),对照组的NAA/(Cho+Cr)值略高于参考文献的值;对照组左、右侧海马结构的体积平均值与参考文献中的体积平均值经方差分析、校正t检验后,右侧P〉0.05,左侧P〈0.05,右侧海马结构体积的平均值对照组的与参考文献的统计学上无显著性差异,左侧海马结构体积的平均值对照组的与参考文献的统计学上有显著性差异,对照组的平均值低于参考文献的平均值。结论:疑诊颞叶癫痫的病人应优先选择行-1H-MRS检查,本研究所做的正常对照组的海马结构体积值以及NAA/(Cho+Cr)比值可以作为癫痫诊断的参考标准。
Objective:Compare the diagnostic value of volumetric MRI and1 H-MRS in order to guide epilepsy patients to choose valuable diagnosis method. Methods:50Temporal Lobe Epilepsy patients confirmed by EEG and clinical materials. Do volumetric MRI and Magnetic Resonance Spectroscopy examinations in bilateral hippocampal structure with 3. 0T MRI equipments. Analyse the respective diagnose accordance rate of two methods, then do a chi-square test. Moreover 43 normal volunteers do volumetric MRI, and1 H-MRS with the 3. 0 T MRI equipments and serve as a control group,then do variance analysis and T test between obtained results of control group and normal value of this paper's references. Results:Calculate respective diagnose accordance rate of Volumetric MRI and1 H-MRS, after a chi-square P〈0. 05, two methods 's diagnose accordance rate has obvious difference. Control group's NAA/( Cho +Cr) normal value and references's NAA/( Cho +Cr) normal value go through variance analysis,and T test,NAA/( Cho+Cr) normal value have no difference in the control group between,P〉0. 05,control group's NAA/(Cho + Cr)normal value is slightly higher than the average value of references;The control group 's left and right hippocampal volume and rereference's volume go through variance analysis and correction t test,Right P〉0. 05,Left P〈0. 05, the hippocampal structural volume 's mean value between control group and references has no difference in the right hippocampal structural, has difference in the left, in the left hippocampal structural control group's is lower than references's. Conclusion:Suspicious Temporal Lobe Epilepsy patients should give preference to ( 1 H-MRS ) , our study get hippocampal structure volume 's normal value and NAA/( Cho+Cr) normal value which can be used as the reference standards for epilepsy di-agnosis.
