肺炎链球菌血清学、PFGE型别与流行和耐药的关联性分析 被引量:9

摘要 目的通过研究肺炎链球菌血清学、耐药脉冲均凝胶电泳(PFGE)关联性,完善和补充区域性肺炎链球菌传播及流行的鉴定标准和预警体系。方法随访调查2012-2015年四川省泸州市人民医院肺炎链球菌流行性,并通过免疫学、血清生化和分子生物学手段鉴定和分析肺炎链球菌的感染特性,进一步分析细菌种类及耐药的相关性。结果 (1)流行发现,儿童(≤14岁)患者的比例(68.3%)明显高于青少年及成人(>14岁)患者(31.7%),无性别差异;7种血清分型结果中,19型占比最高(66.67%),13型和15型最少。(2)血清型与耐药相关分析,发现血清3型的耐药率最高。(3)PFGE带型与耐药相关分析,17种PFGE带型,总耐药率达到75.83%,其中5种药物耐药及SPS16.CN011耐药比率最高。结论该院2012-2015年流行的肺炎链球菌主要发生于儿童(≤14岁)患者,PFGE型对肺炎链球菌的鉴别和诊断优于血清学方法;但对其耐药性分析发现,血清型与耐药性关联作用大于PFGE分型,这可能与细菌本身的致病物质荚膜抗原密切相关。 Objective To study the correlation among drug resistance,serology and PFGE of pneumonias Streptococcus for per- fecting and replenishing the identification standard and early warning system of regional spread and prevalence of pneumonias Strep- tococcus. Methods The prevalence of Streptococcus pneumoniae in our hospital was performed the follow up investigation during 2012-2015 ,its infectious characteristics were identified and analyzed by using the immunology, serum biochemistry and molecular biology means,then the correlation between bacterial species and drug resistance was further analyzed. Results (1) The prevalence found that the proportion of children patients (414 years old) was significantly higher than that of adolescence and adults (〉14 years old) patients (68.3% vs. 31.7%) ,without gender difference; in 7 kinds of serotyping results,the proportion of type 19 was highest (66.67%),which of type 13 and 15 were minimal. (2) The correlation analysis of serotype and drug resistance found that the drug resistance rate of serotype 3 was highest. (3) In the correlation analysis of PFGE band type and drug resistance,the total drug resistance in 17 kinds of PFGE band type reached 75.83% ,in which the resistance rates of five drugs and SPS16. CN011 were highest. Conclusion The infection of pneumonias Streptococcus in this hospital during 2012-2015 primarily occurred among chil- dren (414 years old). The PFGE type for the identification and diagnosis of Streptococcus pneumoniae is superior to the serological method; but its drug-resistance analysis finds that the correlation between the serotype and drug resistance is greater than that of PFGE typing,which may be closely related with the capsule antigen as the pathogenic material of bacteria themselves.
