摘要
目的:探讨首次口腔护理时间对气管插管患者早发性呼吸机相关肺炎(VAP)的影响。方法:随机将2012年1月~2016年10月在我院行经口气管插管术的180例患者等分为A组、B组、C组,三组患者口腔护理首次介入时间与气管插管之间的时间间隔分别为≤4 h、5~7 h、≥8 h,现对比分析各组患者的效果。结果:三组患者机械通气时间、插管后住院时间、早发性VAP发生率相比差异有统计学意义(P<0.05),A组患者机械通气时间、插管后住院时间明显低于B组、C组患者(P<0.05),B组患者机械通气时间、插管后住院时间明显低于C组患者(P<0.05),C组患者早发性VAP发生率明显高于A组、B组患者(P<0.05),A组与B组患者之间早发性VAP发生率相比差异无统计学意义(P>0.05)。三组患者死亡率相比差异无统计学意义(P>0.05)。结论:气管插管后应尽早给予患者口腔护理,这不仅可以缩短患者机械通气时间、插管后住院时间,还可以降低患者早发性VAP发生率。
Objective :To investigate the effect of first oral nursing time on early ventilator - associated pneumonia (VAP) in patients with tracheal intuba-tion. Methods : A total of 180 cases admitted in our hospital from January 2012 to October 2016 were randomly divided into group A, group B and group C. The time interval between first intervention time and tracheal intubation among the three groups were less than 4 hours, 5 to 7 hours and above 8 hours. Then the effects on patients in each group were compared. Results: The incidence of mechanical ventilation, the time of admission after intubation and the incidence of early - onset VAP were statistically significant(P 〈 0. 05). The time of hospitalization and machinery ventilation time of group A were signifi-cantly lower than that of group B and group C(P 〈0. 05). The time of hospitalization and machinery ventilation time of group B were significantly lower than that of group C(P 〈0. 05). The incidence of early - onset VAP in group C was significantly higher than that in group A and group B(P 〈0. 05). The incidence of early - onset VAP in group A and group B showed no statistic difference(P 〉 0. 05 ). There was no statistically significant difference in mortali-ty between the three groups(P 〉0. 05). Conclusion :Oral care should be given as soon as possible after tracheal intubation. It can not only shorten the time of mechanical ventilation and hospitalization time, but also can reduce the incidence of early onset VAP of the patients.
