无创呼吸机联合常规方法治疗COPD 引起的肺心病导致急性肺水肿的临床研究 被引量:2

摘要 目的探讨无创呼吸机联合常规方法治疗慢性阻塞性肺疾病(COPD)引起的肺心病导致急性肺水肿的临床效果。方法选取在我院进行治疗的COPD引起的肺心病导致急性肺水肿的患者80例,随机分为两组,各40例。对照组实施常规方法,观察组在对照组基础上实施无创呼吸机治疗。结果治疗后,观察组心率(HR)、呼吸(RR)、二氧化碳分压(PaCO_2)较对照组明显降低,PH、动脉血氧分压(PaO_2)、血氧饱和度(SaO_2)明显升高,差异具有统计学意义(P<0.05)。观察组住院时间少于对照组,差异具有统计学意义(P<0.05)。观察组住院费用显著低于对照组(P<0.05)。结论给予COPD引起的肺心病导致急性肺水肿患者无创呼吸机联合常规治疗,可促进患者临床症状改善,减轻心肺功能损伤,提高呼吸舒适度,具有临床推广价值。 Objective To explore the clinical effect of noninvasive ventilator combined with routine method in treatment of acute pulmonary edema caused by pulmonary heart disease resulting from chronic obstructive pulmonary disease (COPD). Methods 80 patients with acute pulmonary edema caused by pulmonary heart disease resulting from COPD who were treated in our hospital were randomly divided into two groups,with 40 eases in each.The control group was given routing method,while the observation group was treated with noninvasive ventilator at the basis of the control group. Results After treatment,the heart rate iHR),respiratory rate (RR),partial pressure of carbon dioxide (PaCO2) in the observation group were significantly reduced compared with those in the control group while PH,arterial partial pressure of oxygen (PaO2) and oxygen saturation (SaO2) in the observation group were significantly increased compared to those in the control group.Difference had statistical significance (P 〈 0.05).The hospitalization time of the observation group was shorter than that of the control group and the difference had statistical significance (P 〈 0.05). In addition,hospitalization cost of the observation group was significantly lower than that of the control group (P 〈 0.05). Conclusion Noninvasive ventilator combined with routine method in treatment of patients with acute pulmonary edema caused by pulmonary heart disease resulting from COPD Can promote the improvement of clinical symptoms,reduce the injury to heart and lung function and improve respiratory comfort,which is worthy of clinical promotion.
