摘要
目的探讨记忆合金肋骨接骨板与爪形肋骨接骨板治疗多发肋骨骨折的效果。方法 81例行手术内固定治疗的胸部外伤致多发肋骨骨折患者,随机分为观察组(42例,采用记忆合金肋骨接骨板内固定治疗)和对照组(39例,采用爪形肋骨接骨板内固定治疗),比较两组围手术期指标及术后并发症情况。结果两组患者手术均成功,无术中意外。两组患者术中固定肋骨数、术后胸引管引流时间、术后住院时间比较差异均无统计学意义(P>0.05);观察组手术时间(122.29±21.44)min短于对照组(132.05±21.00)min,术中失血量(175.48±34.30)ml少于对照组(196.41±39.24)ml,术后首次下床活动时间(3.21±1.46)d短于对照组(4.05±1.02)d,差异均具有统计学意义(P<0.05)。两组患者术后切口感染、肺不张、胸廓畸形愈合发生情况比较差异无统计学意义(P>0.05);观察组术后肺部感染、骨折不愈合发生情况(3、0例)均少于对照组(9、4例),差异均具有统计学意义(P<0.05)。所有患者的术后并发症经保守治疗均痊愈。结论采用记忆合金肋骨接骨板治疗多发肋骨骨折治疗效果优于爪形肋骨接骨板,值得临床医师推广应用。
Objective To explore the effect of memory alloy rib bone plate and claw-like rib bone plate in the treatment of multiple rib fracture. Methods A total of 81 chest trauma induced multiple rib fracture patients treated with internal fixation surgery were randomly divided into observation group （42 cases, receiving memory alloy rib bone plate for internal fixation） and control group （39 cases, receiving claw-like rib bone plate for internal fixation）. Comparison were made on perioperative indicators and postoperative complications in two groups. Results Both groups had successful operation without accident during operation. Both groups had no statistically significant difference in intraoperative fixed rib number, postoperative chest tube drainage time and postoperative hospital stay time （P〉0.05）. The observation group had shorter operation time as （122.29 ±21.44） min than （132.05 ± 21.00） min in the control group, less intraoperative bleeding volume as （175.48± 34.30） ml than （196.41± 39.24） ml in the control group, and shorter postoperative first time off-bed activity time as （3.21 ±1.46） d than （4.05 ± 1.02） d in the control group. Their difference had statistical significance （P〈0.05）. Both groups had no statistically significant difference in occurrence of postoperative incision infection, pulmonary atelectasis and thoracic deformity healing （P〉0.05）. The observation group had less occurrence of postoperative lung function and nonunion （3 and 0 ease） than the control group （9 and 4 cases）, and their difference had statistical significance （P〈0.05）. All complications in patients were cured after preservative treatment. Conclusion Implement of memory alloy rib bone plate shows better therapeutic effect than claw-like rib bone plate, and it is worth promotion and application of clinicians.
