摘要
目的观察连续性血液滤过(CBP)对重症脓毒症患者营养状况及炎症指标的影响。方法选取河北省胸科医院于2014年8月—2016年3月收治的重症脓毒症118例,按治疗方法分为观察组和对照组,每组59例,对照组采用常规治疗,观察组在对照组基础上应用CBP治疗,比较2组临床转归指标,治疗前、治疗后1、2周各炎性因子及营养指标水平。结果观察组ICU停留时间、多器官功能衰竭发生率和病死率均短于或低于对照组(P<0.05);治疗后1、2周,观察组肿瘤坏死因子-α、白介素-1β、白介素-6、白介素-8水平低于对照组和治疗前,白蛋白、前白蛋白、转铁蛋白及胰岛素样生长因子-1水平高于治疗前和对照组(P<0.05,P<0.01)。结论 CBP能提高重症脓毒症患者的疗效,改善患者营养状况。
Objective To observe effect of continuous hemofiltration on nutritional status and inflammatory indexes of patients with severe sepsis. Methods A total of 118 patients with severe sepsis admitted during August 2014 and March 2016 were divided into control group and observation group （n = 59 for each group） according to different treatment methods. Control group was treated with conventional treatment, while observation group was added continuous hemofihration on the basis of treatment for control group. Clinical turnover indexes, levels of inflammatory factors and nutritional indexes before and 1 and 2 weeks after treatment were compared in two groups. Results In observation group, ICU detention time, incidence rate of multiple organ failure （MOF） and mortality were significantly lower than those in control group （P 〈 0. 05）. In observation group, 1 and 2 weeks after treatment, levels of tumor necrosis factor-α（TNF- α）, interleukin-1β（ IL-1β）, IL-6 and IL-8 were significantly lower （P 〈 0. 01 ）, while levels of albumin, prealbumin, transferrin and insulin-like growth factor-1 （ IGF-1 ） were significantly higher than those before treatment and in control group （P 〈0. 05）. Conclusion Continuous hemofiltration can improve efficacy and nutritional status of patients with severe sepsis.
