原发性急性闭角型青光眼脉络膜厚度变化及相关影响因素 被引量:4

Changes and Related Factors of Choroidal Thickness in Patients with Primary Acute Angle-closure Glaucoma

摘要 目的探讨原发性急性闭角型青光眼(APACG)脉络膜厚度变化及相关影响因素。方法选取2015年1月—2015年12月在本院眼科门诊就诊的120例原发性急性闭角型青光患者,另选取同期行常规眼科检查的120例健康体检者为对照组,应用谱域相干断层深度增强扫描(EDI SD-OCT)测定两组脉络膜厚度,应用单因素及Logistic多因素分析影响APACG患者脉络膜厚度变化相关危险因素。结果APACG组脉络膜厚度、中心凹下脉络膜厚度较对照组薄(P<0.001),两组眼轴长度、玻璃体长度、屈光度、前房深度、晶体厚度差异有统计学意义(P<0.001)。经Pearson单因素分析显示,年龄、眼轴、屈光度与APACG组中心凹下脉络膜厚度呈负相关(r=-0.412,-0.358,-0.367,P<0.05),与前房深度呈正相关(r=0.429,P<0.05)。经Logistic分析显示,年龄、眼轴、屈光度数是影响APACG脉络膜厚度变薄的独立危险因子。结论APACG脉络膜厚度变化与部位有关,年龄、眼轴、屈光度数与APACG患者中心凹下脉络膜呈负相关,且是APACG脉络膜变薄的独立危险因素。 Objective To investigate the changes and related factors of choroidal thickness of patients with acute primary angle-closure glaucoma(APACG).Methods A total of 120 patients with APACG(the APACG group)treated in the Ophthalmology Clinic between January 2015 and December 2015,and another 120 healthy people receiving routien eye examinations(the control group)at the same time were selected for this study.The choroidal thickness of the two groups was measured with enhanced spectral domain OCT depth scanning(EDI SD-OCT).Risk factors related to choroidal thickness variations were analyzed with single factor and logistic multivariate.Results The levels of choroidal thickness,subfoveal choroidal thickness of the APACG group were below those of the control group(P〈0.05),and axial length,vitreous length,refraction,anterior chamber depth,lens thickness were significantly different between the two groups(P〈0.05).Age,axial length and refraction in the APACG group were negatively correlated with subfoveal choroid thickness by univariate analysis(r=-0.412,-0.358,-0.367,P〈0.001),while the anterior chamber depth was positively correlated with subfoveal choroid thickness(r=0.429,P〈0.001).Logistic regression analysis showed that age,axial length and refraction were independent risk factors of choroidal thickness thinning in patients with APACG.Conclusion APACG choroidal thickness variations are related to the sites of disease.Age,axial length,refraction and subfoveal choroid thickness in APACG patients are not only negatively correlated,but independent risk factors of APACG choroidal thinning.
