摘要
目的 探讨股骨近端抗旋髓内钉置入对老年骨质疏松性股骨转子间骨折患者近、远期疗效的影响.方法 回顾性分析65例老年股骨转子间骨折患者的临床资料,其中行股骨近端抗旋髓内钉固定（PFNA）的40例患者作为研究组,行动力髋螺钉固定（DHS）的25例患者作为对照组,随访1年,比较两组近、远期疗效,手术时间,切口长度,术中出血量,骨折愈合时间及随访期间并发症发生情况.结果 两组患者术后1个月及随访1年疗效等级比较差异有统计学意义（P〈0.05）,组内比较差异未见统计学意义（P〉0.05）;研究组手术时间、术中出血量、切口长度、骨折愈合时间显著优于对照组（P〈0.05）,且前者并发症发生率为2.50%,显著低于后者（20.00%,P〈0.05）.结论 与DHS内固定比较,PFNA内固定近、远期疗效显著,创伤小,术后并发症少,有利于老年骨质疏松性股骨转子间骨折患者术后恢复,具有良好的应用前景.
Objective To investigate the effect of modified placement of proximal femoral nail anti rotation on the short-term and long-term efficacy in elderly patients with osteoporotic femoral intertrochanteric fractures.Methods The clinical data of 65 elderly patients with osteoporotic femoral intertrochanteric fractures were retrospectively analyzed, 40 patients who were treated by placement of proximal femoral nail anti rotation（PFNA） were selected as the study group, and the other 25 patients who were treated by dynamic hip screw fixation（DHS） were selected as the control group.Followed up for 1 year, the short-term and long-term efficacy, operation time, incision length, intraoperative blood loss, fracture healing time and complications during follow-up were compared between the two groups.Results The differences in the level comparison of efficacy between the two groups after 1 month of operation and during follow-up were significant（P〈0.05）, but comparison within the groups, the differences were not significant（P〈0.05）.The operation time, intraoperative blood loss, incision length and fracture healing time in the study group were significantly better than those in the control group（P〈0.05）, and the incidence rate of complications of the study group was 2.50%, which was significantly lower than that of the control group（20.00%,P〈0.05）.Conclusions The short-term and long-term efficacy of PFNA is remarkable compared with that of DHS, with small trauma, less postoperative complication, is helpful for postoperative recovery of elderly patients with osteoporotic intertrochanteric fractures, which has good application scenarios.
