摘要
目的探讨应用经皮微创接骨板内固定(MIPO)治疗肱骨干中段骨折患者的术后骨代谢指标及临床疗效。方法将108例肱骨干中段骨折患者按照治疗方法分成两组,对照组(48例)采用传统切开复位内固定,观察组(60例)采用MIPO治疗。比较两组骨代谢指标及临床疗效。结果患者均获得随访,时间6个月。观察组术后重度疼痛例数少于对照组,轻度疼痛例数多于对照组,差异均有统计学意义(P<0.01)。观察组术后骨代谢指标(CTX、PICP、PTH与BGP)水平均优于对照组,差异均有统计学意义(P<0.01)。术后两组Mayo与Constant-Murley功能评分均得到改善(P<0.05),观察组较对照组改善幅度更明显(P<0.05)。结论MIPO治疗肱骨干中段骨折患者可减轻疼痛,优化患者骨代谢指标水平,促进患者关节功能恢复。
Objective To study the effect of minimally invasive plate osteosynthesis（ MIPO） on the postoperative bone metabolic index and clinical effects in patients with humeral midshaft fractures. Methods The clinical data of108 patients with humeral midshaft fractures were divided into two groups according to the different clinical treatment plans given. The control group（ n = 48） was treated with traditional open reduction and internal fixation,whereas the observation group（ n = 60） was treated with MIPO. The bone metabolic index and clinical effects in the two groups were compared. Results A six-month follow-up was got in both groups. The proportion of patients suffering from severe pains in the observation group was less than that in the control group,the observation group cases of mild pain was more than the control group,where statistically significant difference was registered（ P〈0. 01）. The levels of bone metabolic markers（ CTX,PICP,PTH and BGP） were shown to be better than those in the control group,with statistically significant difference registered（ P〈0. 01）. Scores of Mayo and Constant-Murley functions were improved in both groups（ P〈0. 05）,and the improvement in the observation group was more significant,where statistically significant difference was registered（ P〈0. 05）. Conclusions For patients with humeral midshaft fractures,MIPO can relieve the patients＇ pains,optimize the level of bone metabolic markers and promote their recovery of joint function.
