摘要
目的比较3种常见方法治疗儿童肱骨髁上骨折的临床疗效。方法以186例GartlandⅡ、Ⅲ型伸直型肱骨髁上骨折儿童患者为研究对象,根据治疗方法分为闭合复位石膏固定组(n=44)、闭合复位经皮穿针内固定组(n=74)和切开复位克氏针固定组(n=68)。记录3组患者骨折并发症发生情况,根据Flynn标准评价3种方法治疗后患肢的功能,比较3种治疗方法的临床疗效。结果患者均获得1年随访。并发症发生率:闭合复位经皮穿针内固定组和切开复位克氏针固定组显著低于闭合复位石膏固定组(P<0.001)。临床治疗优良率:闭合复位经皮穿针内固定(94.6%)和切开复位克氏针固定组(95.6%)明显高于闭合复位石膏固定组(65.9%)(P<0.001),闭合复位经皮穿针内固定组与切开复位克氏针固定组比较差异无统计学意义(P>0.05)。结论闭合复位经皮穿针内固定和切开复位克氏针内固定是治疗儿童肱骨髁上骨折较好的方法。
Objective To compare the effects among three common orthopaedic procedures in the treatment of humeral condylar fracture in children. Methods Total of 186 patients with Gartland type Ⅱ or Ⅲ extension supracondylar humeral fractures were prospectively included as study subjects. According to the treatment methods,petients were divided into closed reduction and plaster immobilization group( n = 44),closed reduction and percutaneous pin fixation group( n = 74) and open reduction with Kirschner wire fixation group( n = 68). The fracture complications in three groups were recorded,and according to the Flynn standard,the clinical curative effect was compared among three methods. Results Children were followed up for 1 year. The complication incidence rate in closed reduction and percutaneous pin fixation group and open reduction with Kirschner wire fixation group were significantly lower than closed reduction and plaster immobilization group( P〈0. 001). The clinical treatment excellent-good rate in closed reduction and percutaneous pin fixation group( 94. 6%) and open reduction with Kirschner wire fixation group( 95. 6%) were significantly higher than closed reduction and plaster immobilization group( 65. 9%)( P〈0. 001),but there was no significant difference between closed reduction and percutaneous pin fixation and open reduction with Kirschner wire fixation( P〈0. 05). Conclusions Closed reduction and percutaneous pin fixation and open reduction with Kirschner wire fixation are good methods to treat supracondylar fracture of humerus in children.
