摘要
目的 探讨抑癌基因APC蛋白在大肠癌发生及演进的作用。方法 应用免疫组织化学方法分析了大肠腺癌、大肠腺瘤及腺瘤癌变和正常大肠粘膜中APC蛋白的表达。结果 正常粘膜中APC蛋白阳性率为100%(45/45),大肠腺瘤及腺瘤癌变中阳性率分别为80%(20/25)和55.5%(6/11),大肠腺癌中APC蛋白阳性率为44.4%(40/90),经统计分析发现大肠腺癌及腺瘤癌变APC蛋白阳性率显著低于正常粘膜(P<0.01),大肠腺癌中APC蛋白阳性率也显著低于大肠腺瘤(P<0.01),而大肠腺癌与腺瘤癌变之间以及腺瘤与正常粘膜之间的APC蛋白阳性率无显著差别(P>0.05),APC蛋白表达缺失与腺瘤类型、大小及组织学分级无关,与大肠癌临床病理参数无关。结论 APC蛋白表达缺失可能在大肠癌的早期发生中起重要作用。
Objective To explore the role of antioncogene APC protein in the development and progress of colorectal adeno-carcinoma( CRC). Methods Expression of APC protein was examined with immunohistochemical technique in 45 cases of colorectal mucosa,25 cases ofadenoma.il cases of canceration adenoma and 90 cases of CRC. Results The positive expression rate of APC protein gradually reduced as the negative expression or loss of expression APC protein increased in the progress of development from normal colorectal mucasa to adenoma and carcinoma. APC protein was found in 45/45 ( 100% ) cases of normal colorectal mucosa , 20/25 (80% ) cases of adenoma and 6/11 (55. 5% ) cases of malignant change type of adenoma respectively. It was detected only in 40/90(44. 4% ) cases of CRC. It was found that the positive rate APC protein in CRC and malignant type of adenoma was significantly lower than that of normal mucosa(P < 0. 01 ) . The expression rate was also significantly lower in CRC than in adenoma ( P < 0. 01). There was no significant difference in APC protein expression between CRC and malignant change type of adenoma as well as between adenoma and the normal mucosa (P > 0. 05 ) . No significant correlation was found between the expression loss of APC protein and pathological type. size and staging of adenoma, neither was done between the loss of expression and any pathological parameters of CRC. Conclusions Expression loss of APC protein might play a role in early carcinogenesis of CRC.
