摘要
目的探讨我院住院病人急性肾损伤(AKI)的特点及预后,为早期建立AKI预防体系、改善预后提供依据。方法选取2011年6月—2016年6月我院住院发生AKI病人1 432例,分析AKI的发生率、病因、科室分布、分期及预后。结果我院住院病人AKI的发生率为3.61%(1 432/39 643)。AKI病人男女比例为2.02∶1,平均年龄为(62.43±16.72)岁。肾前性AKI最多,占62.4%,具体病因感染居首位(26.6%),其次为低血容量(24.2%),再次为药物性肾损伤(18.7%)。内科病人占59.1%,以ICU最常见(33.7%),其次为心内科(8.5%)和血液科(5.7%);外科病人常见于普外科(14.1%)和神经外科(6.8%)。病人年龄越大,AKI发生率越高,病死率越高;分期越高,恢复的可能性越低,进入终末期或死亡的风险越大。结论我院住院病人的AKI较常见,好发于男性,以肾前性多见,感染、低血容量及药物为主要病因,预后与年龄、分期、病因相关。
Objective To investigate the characteristics and prognosis of inpatients with acute kidney injury（AKI）in our hospital,and provide the basis for establishing prevention system and improving prognosis. Methods This study consisted of1 432 AKI in hospitalized patients of our hospital-during June 2011-June 2016 period.The incidence,etiology,distribution,staging and prognosis were analyzed. Results The incidence of AKI in hospitalized patients 3.61%（1 432/39 643）,and the ratio of male to female was 2.02∶1,with an average age of（62.43±16.72）years.Prerenal AKI was the most common,accounting for62.4%,the specific cause of infection headed the first（26.6%）,followed by low blood volume（24.2%）,and drug-induced renal damage（18.7%）.Medical patients accounted for 59.1%,ICU was the most common（33.7%）,followed by Departments of Cardiology（8.5%）and Hematology（5.7%）.As for surgical patients,they were usually seen in Department of General Surgery（14.7%）and Department of Neurosurgery（6.8%）.The older the patient＇s age,the higher the incidence of AKI,and the higher the mortality.The higher the stage was,the lower the likelihood of recovery,and the greater the risk of entering the final stage or death. Conclusion Acute kidney injury is more commonly seen in inpatients of our hospital,more common in men with pre-renal causes,the main causes are infection,low blood volume and drugs,the prognosis is related to patient＇s age,staging and etiology.
