摘要
目的探讨腰椎退行性变核素骨显像与CT退变程度和部位的相关性。方法回顾性分析120例腰椎退行性变病人核素骨显像与CT表现,对CT显示腰椎退变程度进行分类、记录,并观察对应的腰椎核素浓聚程度、形态,分析腰椎退行性变核素浓聚程度、形态与CT腰椎退变程度和部位的相关性。结果 Spearman相关分析显示,腰椎退行性变核素骨显像核素异常浓聚程度与椎体骨质增生程度、椎小关节增生硬化程度、终板硬化程度、椎间盘膨出或突出程度相关(r=0.152~0.588,P<0.05)。腰椎核素异常浓聚呈特定形态"倒八字征"8个,"眼征"7个,皆有双侧椎小关节Ⅲ度增生CT表现。结论腰椎退行性变核素浓聚程度与CT腰椎退变程度有关,"倒八字征"或"眼征"是腰椎小关节退变核素骨显像一种特定表现形态。
Objective To investigate the association of the image features of lumbar spine degeneration（LSD）on radionuclide bone imaging with the location or severity of LSD on CT. Methods Radionuclide bone imaging and CT manifestations of120 patients with LSD were retrospectively reviewed.The degree of LSD on CT was classified and recorded,and the concentration and morphology of the lumbar nucleus were observed.The correlation between lumbar nucleus concentration and morphology and the degree and location of LSD on CT was analyzed. Results Spearman correlation analysis showed that abnormal concentration of radionuclide bone imaging was correlated with the degree of vertebral bone hyperplasia,the degree of vertebral facet joint hyperplasia and sclerosis,the degree of hardening of the end plate,and the degree of slipped disc（r=0.152-0.588,P〈0.05）.The abnormal concentration of radionuclide bone imaging showed in a particular form as ＂a reversed syndrome＂ in eight vertebral bodies and ＂eye sign＂ in seven vertebral bodies all with third degree hyperplasia of facet on CT. Conclusion The degree of nuclide accumulations in LSD is associated with the degree of LSD on CT.＂A reversed syndrome＂ or ＂eye sign＂ is a specific form of expression of radionuclide bone imaging in lumbar facet joint degeneration.
