摘要
目的探讨用西格玛性能验证图评价常规生化检测项目,结合质量目标指数(QGI)查找性能不佳原因,运用Westgard西格玛多规则选择最佳质量控制规则,指导检验质量改进。方法收集解放军第一六三医院检验科2016年全国室间质量评价常规生化23个检验项目,使用医学信息网提供的软件操作,计算σ度量值和绘制西格玛性能验证图,评价检验项目分析性能。结果 23个常规生化检测项目中,6个项目的分析性能>6σ;4个项目分析性能5≤σ<6;1个项目分析性能4≤σ<5;2个项目分析性能3≤σ<4;10个项目分析性能<3σ。其中,σ度量值最大的是肌酸激酶,为10.65;最小的是尿素氮,为1.08;平均σ度量值为4.53。在未达到6σ的17个检验项目中,采用计算质量目标指数来查找性能不佳原因,有14个检测项目需要优先改进精密度,1个项目需要优先改进准确度,2个项目精密度和准确度都需要改进。结论将西格玛性能验证图和质量目标指数有机结合,可以评价检测项目性能和查找性能不佳原因,有利于实验室项目持续改进。
Objective To investigate the evaluation of routine chemical detection project with sigma verification of performance chart,combined with the quality goal index(QGI) to find the cause of poor performance,using the Westgard sigma rules to choose the best quality control,and to guide the inspection quality continuous improvement.Methods The date were collected from the National External Quality Assessment of routine chemical 23 test items in the 163 rdHospital of PLA during 2016.The performance of the test items was evaluated by using the software operation provided by the medical information network,calculating the sigma value and drawing the sigma of verification performance chart.Results Among 23 routine chemical testing projects,the analysis performance of the six projects was 〉6 sigma,4 project analysis performance was 5≤σ 6,one project analysis performance was 4≤σ 5,2 analysis performance was 3≤σ 4; 10 projects analysis performance was〈 3 sigma,Among them,the maximum value of σ is CK,10.65; the smallest is BUN,1.08; the average value of σ is 4.53.In the 17 test items that did not meet 6σ,the calculation of the quality goal index was used to find the cause of poor performance,there are 14 test items need to improve the accuracy of priority,a project needs to improve the accuracy of priority,two projects need to improve the precision and accuracy.Conclusion The combination of the sigma of verification performance chart and the quality goal index can be used to evaluate the performance of the test items and to find out the reasons for poor performance,which is conducive to the continuous improvement of laboratory projects.
