摘要
目的前列地尔联合鼓室内注射地塞米松治疗突发性耳聋的临床疗效分析。方法选取2015年10月—2016年8月间黑龙江省绥化市第一医院收治的突发性耳聋患者58例,随机分为两组,对照组和实验组各29例。对照组和实验组的患者均接受常规治疗,除此之外,实验组患者给予鼓室内注射地塞米松1 mL/d,患者的患耳保持向上30 min,禁止吞咽动作。对两组患者的临床治疗效果(分为治愈、显效、有效、无效)进行分析,判定标准为:治愈,患者受损频率恢复正常,达到健耳水平,或该患者健康水平;显效,患者受损频率平均提高30 d BHL以上;有效,患者受损频率平均提高15~30 dBHL;无效,患者受损频率平均提高低于15 dBHL;同时对所有患者的空气传导听阈值、血液流变学指标进行统计。结果实验组患者的总有效率为96.55%,明显高于对照组患者,同时,实验组患者的空气传导听阈值与对照组患者的空气传导听阈值相比下降幅度更大。其中实验组听阈值平均下降23.22,下降幅度为33.98%,对照组听阈值平均下降9.48,下降幅度为13.89%。结论前列地尔联合鼓室内注射地塞米松治疗突发性耳聋的治疗效果较好,患者能及时恢复听力,应在临床中推广使用。
Objective To analysis the clinical curative effect of alprostadil and intratympanic injection of hexadecadrol in treatment of sudden hearing loss. Methods 58 cases of patients with sudden hearing loss admitted and treated in Suihua first hospital from October 2015 to August 2016 were selected and randomly divided into two groups with 29 cases in each, both groups adopted the routine treatment, the experimental group were given intratympanic injection of 1ml hexadecadrol, Decision criteria for cure, the patient dam-age frequency back to normal, healthy ear level, or the patients health level; Pay off, in patients with damage to the average frequency increased by more than 30 dBHL; Effectively, an average increase of patients with damaged frequency 15-＊30 dBHL; Invalid and dam aged frequency increase on average is below 15 dBHL patients and the clinical treatment effect （cured, markedly effective, effective and ineffective） were compared between the two groups, and the aerotympanal conduction hearing threshold value and hemorheology of all patients were counted. Results The total effective rate in the experimental group was 96.55%, which was obviously higher than that in the control group, at the same time, the aeiotympanal conduction hearing threshold value in the experimental group decreased more obviously in the control group （23.22, 33.98% vs 9.48, 13.89%）. Conclusion The effect of alprostadil and intratympanic injection of hexadecadrol in treatment of sudden hearing loss is better, and the hearing of patients can recover in time, which can be promoted and applied in clinic.
