摘要
目的探讨血清补体C3c、C4、CH50及免疫球蛋白IgG、IgA、IgM检测在系统性红斑狼疮(SLE)诊断中的应用价值。方法采用速率散射比浊法检测75例SLE患者及87例对照者(病例对照组65例,健康对照组22例)血清中C3c、C4、CH50、IgG、IgA和IgM水平并进行比较,同时采用受试者工作特性曲线(ROC)分析各指标对SLE的诊断性能并进行比较与评价。结果 SLE组的C3c、C4及CH50水平均显著低于病例对照组及健康对照组(P<0.01),各组间IgG、IgA、IgM水平差异未发现统计学意义(P>0.05)。仅C3c、C4及CH50对SLE具有诊断价值(Z=10.279,P<0.01;Z=6.149,P<0.01;Z=7.715,P<0.01),其中C3c检测SLE的曲线下面积(AUC)显著优于C4[0.829(0.762~0.883)vs 0.744(0.670~0.810),Z=2.620,P<0.01],C3c、C4与CH50诊断SLE的灵敏度与特异性的差异均无统计学意义(X^2=3.533,P=0.171;X^2=2.097,P=0.351)。结论 SLE患者存在血清补体C3c、C4和CH50水平降低的现象;血清C3c、C4和CH50对SLE具有一定的诊断价值。
Objective To discuss the application value of serum complements (C3c, C4, CH50) and immunoglob- ulins (IgG, IgA, IgM) in the diagnosis of systemic lupus erythematosus (SLE). Methods The levels of the Serum C3c, C4, CH50, IgG, IgA and IgM in 75 patients with SLE were compared with those of 87 controls by rate scatter turbidimetry (including 65 patients with SLE, 22 healthy people as controls). Meanwhile, the indexes of diagnostic accuracy were com- pared and evaluated by receiver operation curve. Results The levels of C3e, C4 and CH50 were significantly lower in the SLE group than those in the other control groups and healthy groups (P 〈 0. 01 ). No statistical significance was found in IgG, IgA and IgM in the groups (P〉0. 05). But C3c, C4 and CHS0 showed diagnostic value in SLE (Z = 10. 279, P 〈0. 01 ; Z = 6. 149,P 〈 0. 01 ; Z = 7. 715, P 〈 0. 01 ). C3c, whose area beneath the ROC [ 0. 829 (0. 762 - 0. 883 ) ] was obviously larger than C4 [ 0. 744 (0. 670 - 0. 810) , Z = 2. 620, P 〈 0. 011- No statistical differences were found in sensitivity or spe- cificity in diagnosing SLE by C3c, C4 and CH50 (x^2 =3.533, P=0. 171; x^2 =2.097, P =0.351). Conclusion The C3c, C4 and CH50 levels of serum complements are decreased in the patients with SLE. C3c, C4 and CHS0 have a diagnosis value of SLE.
