摘要
目的研究降钙素原(Procalcitonin,PCT)、C反应蛋白(C-reactionprotein,CRP)、血沉(Erythrocyte sedimentation rate,ESR)联合检测对肺炎旁胸腔积液的诊断意义。方法选取2014年7月至2016年4月至我院呼吸内科住院的48例肺炎旁胸腔积液患者作为观察对象,将其分为UCPE组和CPE组,另选20例漏出液患者作为对照组,依次取血测其PCT、CRP、ESR的水平并记录分析。结果对照组、UCPE组、CPE组三组患者的PCT、CRP、ESR存在统计学差异(P<0.05),UCPE组、CPE组患者的PCT、CRP、ESR值均明显高于对照组(P<0.05),但是UCPE组、CPE组患者的PCT、CRP、ESR值无统计学差异(P>0.05);高风险患者组PCT、CRP、ESR水平均明显高于低风险患者组(P<0.05);低风险组患者的住院时间明显短于高风险组(P<0.05),低风险组患者的并发症发生率以及病死率均明显低于高风险组患者(P<0.05)。结论 PCT、CRP、ESR联合检测能提高肺炎旁胸腔积液的诊断的灵敏度和准确性,值得推广及应用。
Objective To investigate the significances of combination test of procalcitonin（PCT）,C-reaction protein（CRP）,and erythrocyte sedimentation rate（ESR）in diagnosis of parapneumonic pleural effusion.Methods Totally 48 patients with parapneumonic pleural effusion hospitalized in Department of Respiratory Medicine of our hospital between July 2014 and April 2016 were selected and divided into UCPE group and CPE group;another 20 patients with transudate were taken as control group;and the blood levels of PCT,CRP,and ESR were determined,recorded,and analyzed.Results The control group,UCPE group,and CPE group had significantly different PCT,CRP,and ESR（P〈0.05）;the UCPE group and CPE group had significantly higher values of PCT,CRP,and ESR than the control group（P〈0.05）;but the UCPE group and CPE group had insignificantly different values of PCT,CRP,and ESR（P〈0.05）;the high-risk group had significantly higher values of PCT,CRP,and ESR than the low-risk group（P〈0.05）;the low-risk group had significantly shorter length of stay than the high-risk group（P〈0.05）;and the low-risk group had significantly lower complication incidence and fatality rate than the high-risk group（P〈0.05）.Conclusion Combination test of PCT,CRP,and ESR can elevate the sensitivity and accuracy in diagnosis of parapneumonic pleural effusion,and consequently is worthy of application.
