摘要
目的:研究分析实时三维超声心动图(RT-3DE)在心肌梗死合并室间隔穿孔的诊断价值。方法:将2010年1月-2016年12月在本院进行治疗的28例心肌梗死合并室间隔穿孔患者作为研究对象,其中对照组采用二维超声心动图(2DE)进行诊断,观察组采用实时三维超声心动图(RT-3DE)进行诊断,应用不同诊断技术进行诊断后比较研究对象室间隔穿孔病变部位成像质量及诊断的准确率。结果:采用不同方法进行诊断后,观察组的成像质量稍高于对照组,但成像质量相近,差异无统计学意义(P>0.05);观察组诊断符合率100%(14/14)高于对照组诊断符合率78.51%(11/14),差异有统计学意义(P<0.05);观察组患者的心功能指标有异于对照组,能够更为清晰地显示心脏病变情况,差异有统计学意义(P<0.05)。结论:对心肌梗死合并室间隔穿孔患者应用RT-3DE进行诊断,不但能获得较好成像质量,更加直观地显示病变的形态及其与邻近结构的关系,更能准确测量破口大小,RT-3DE彩色显像可清晰、立体显示穿孔的过隔血流,有效提高心脏病变诊断符合率,为临床提供丰富的影像信息,在心肌梗死合并室间隔穿孔的诊断中具有积极的应用价值。
Objective: To study the diagnostic value of real-time three-dimensional echocardiography ( RT-3DE ) in patients with myocardial infarction combined with ventricular septal perforation.Method: From Jan 2010 to Dec 2016 in our hospital 28 cases of patients with ventricular septal perforation combined with myocardial infarction as the research objects, the control group was given two dimensional echocardiography ( 2DE ), and the observation group was diagnosed by RT-3DE, the accuracy rate and imaging quality of ventricular septal perforation using different diagnostic techniques were compared.Result: After using different methods after diagnosis, the imaging quality of the observation group was sightly higher than that of the control group, and there was no statistically significant difference ( P〉0.05 ) .The coincidence rate of diagnosis of the observation group 100% ( 14/14 )was significantly higher than that of the control group 78.51% ( 11/14 ), the difference was statistically significant ( P〈0.05 ) .Cardiac function index of the observation group was different from the control group, can be more clearly showed that patients with heart disease, and the difference was statistically significant (P〈0.05) .Conclusion: Right ventricular myocardial infarction patients with septal perforation by RT-3DE diagnosis, not only can obtain better image quality, more intuitive to show the lesion morphology and its relationship with adjacent structures, can accurately measure the size of the break, RT-3DE color imaging can clear and stereoscopic display blood flow across the perforation, effectively improve the heart disease diagnosis. Providing abundant information for clinical imaging, which has a positive value in the diagnosis of myocardial infarction combined with ventricular septal perforation.
