摘要
目的调查慢性肾脏病患者的营养状况并分析其特点,为进一步的饮食营养干预提供依据。方法收集2015年1月至2016年8月在浙江中医药大学附属广兴医院肾病营养门诊首次就诊的325例慢性肾脏病患者的临床资料,进行统计分析。结果根据改良主观综合营养评估法评分,发生营养不良73例(22.5%),其中慢性肾脏病1期13例、2期10例、3期25例、4期16例、5期9例;消瘦22例(6.8%)、超重和肥胖131例(40.3%)、低蛋白血症71例(21.8%)、贫血116例(35.7%)、高脂血症121例(37.2%);慢性肾脏病4~5期患者肱三头肌皮褶厚度、上臂围明显低于慢性肾脏病1~2期和慢性肾脏病3期患者,比较差异均有统计学意义(P<0.05);血红蛋白随肾小球滤过率下降而显著下降(P<0.05)。结论慢性肾脏病患者营养状况随着肾功能的下降逐渐下降,应及早干预,同时还应重视超重和肥胖、高脂血症等问题。
Objective To investigate nutritional status of patients with chronic kidney disease and analyze its characters to provide evidence for further diet and nutrition intervention.Method Collect the clinical information of 325 patients with chronic kidney disease visiting renal nutrition clinic of Guangxing Hospital Affiliated to Zhejiang Chinese Medical University from Jan.2015 to Aug.2016 for the first time.Analyze the information.Result According to modified quantitative subjective global assessment of nutrition(MQSGA),there are 73 cases of malnutrition(22.5%),in which 13 cases are in stage 1 of chronic kidney disease,10 cases are in stage 2,25 cases are in stage 3,16 cases are in stage 4 and 9 cases are in stage 5.There are 22 cases(6.8%) of emaciation,131 cases(40.3%) of overweight and obesity,71 cases(21.8%) of hypoproteinemia,116 cases(35.7%) of anemia and 121 cases(37.2%) of hyperlipidemia.Triceps skin fold thickness and biceps circumference of patients in stage 4-5 of chronic kidney disease are obviously less than patients in stage 1-2 and 3 of chronic kidney disease.The difference is significant(P〈0.05).Hemoglobin reduces as glomerular filtration rate falls obviously(P〈0.05).Conclusion Nutritional status of patients with chronic kidney disease gets poorer with renal function decreases.Early intervention is necessary and problems of overweight,obesity and hyperlipidemia need more attention.
