2型糖尿病患者大血管病变与踝臂脉搏波传导速度的相关性研究 被引量:15

Relevance between macrovascular disease and brachial-ankle pulse wave velocity in patients with type 2 diabetes mellitus

摘要 目的探讨T2DM患者大血管病变与踝臂脉搏波传导速度(baPWV)的相关性。方法选取T2DM患者284例,根据是否患有T2DM合并大血管病变分为T2DM合并大血管病变组139例和单纯T2DM组145例。检测两组baPWV和其他生化指标。结果两组baPWV、高敏C反应蛋白(hsC-RP)和年龄比较,差异有统计学意义(P<0.05)。相关性分析显示,baPWV异常与DBP、hsC-RP和年龄呈正相关。二元Logistic回归分析显示,DBP、FPG、HbA_1c、hsC-RP和年龄是T2DM合并大血管病变的影响因素,FPG和TC是危险因素。受试者工作特征(ROC)曲线下面积为0.641,当baPWV切点为18.23m/s时,其敏感性与特异性分别为83.5%和43.4%,具有一定的诊断价值。结论 T2DM大血管病变与baPWV呈一定的相关性,可能与hsC-RP的升高对baPWV的影响有关,对T2DM大血管病变具有一定的诊断价值。 Objective To investigate the correlation between macrovascular disease and ankle brachial pulse wave velocity(baPWV)in patients with type 2diabetes(T2DM). Methods 284T2 DM were selected and divided into group with macrovascular disease(n=139)and group without macrovascular disease(n=145).BaPWV and other biochemical parameters were measured and compared between two groups. Results There were significant differences of baPWV,high sensitive C reactive protein(hsC-RP)and age between two group(P〈0.05).Correlation analysis showed that abnormal baPWV was positively correlated with DBP,hsC-RP and age.Logistic regression analysis showed that DBP,FPG,HbA1 c,hsC-RP and age were influence factors of T2 DM complicated with macrovascular disease,FPG and TC were the risk factor.The area under ROC curve was 0.641.When baPWV 18.23m/s was taken as cut off point,the sensitivity and specificity were 83.5% and 43.4%,respectively. Conclusion There is a certain correlation between T2 DM macrovascular disease and baPWV,which may be related to the increase of hsC-RP.
