尿微量蛋白/肌酐比值对冠心病合并2型糖尿病患者心功能及糖、脂代谢的影响 被引量:1

Effect of urinary microprotein/creatinine ratio to cardiac function and glycometabolism in patients with coronary heart disease complicated type 2 diabetes mellitus

摘要 目的探讨尿微量蛋白/肌酐(A/C)比值对冠心病合并2型糖尿病(T2DM)患者心功能及糖、脂代谢的影响。方法选取2015年1月~2017年12月河北省哈励逊国际和平医院心内科收治的826例冠心病合并T2DM患者作为研究对象,按A/C比值分为A/C异常组(n=472)及A/C正常组(n=354)。比较两组患者的心功能参数、血糖与胰岛素抵抗指数(IR)、血脂水平。结果 A/C异常组患者的左心室射血分数(EF%)低于A/C正常组,差异有统计学意义(P<0.05),A/C异常组患者的左心室收缩末期内径(LVESD)及左心室舒张末期内径(LVEDD)高于A/C正常组,差异有统计学意义(P<0.05)。A/C异常组患者空腹血糖(FPG)、餐后2 h血糖(2 h PG)、IR、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)及小而密低密度脂蛋白胆固醇(sd LDL-C)高于A/C正常组,差异有统计学意义(P<0.05),总胆固醇(TC)低于A/C正常组,差异有统计学意义(P<0.05)。结论 A/C比值能够预测冠心病合并T2DM病患者糖、脂代谢水平及心功能状态。 Objective To investigate effect of urinary mieroprotein/ereatinine ratio (A/C) on cardiac function and glyeometabolism in patients with coronary heart disease complicated type 2 diabetes mellitus (T2DM). Methods A total of 826 patients with coronary heart disease complicated T2DM form January 2015 to December 2017 were selected as subjects and were divided into A/C abnormal group (n=472) and A/C normal group (n=354) according to the A/C ratio. The differences of cardiac function parameters, blood glucose, insulin resistance index (IR) and blood lipid levels were contrasted between A/C abnormal group and A/C normal group,and correlation between A/C ratio and cardiac function parameters and glueofipid metabolism was analyzed. Results The left ventrieular ejection fraction (EF%) in the A/C abnormal group was lower than that in the A/C normal group, the difference was statistically significant (P〈0.05).Left ventrieular end systolic diameter (LVESD) and left ventrieular end diastolic diameter (LVEDD) in the A/C abnormal group were higher than those in the A/C normal group, the differences were statistically significant (P〈0.05). The fasting plasma glucose (FPG), 2-hour postprandial blood glucose (2 h PG), IR, triglyeeride (TO, low density fipoprotein cholesterol (LDL-C), small dense low-density lipoprotein cholesterol (sd LDL-C) in the A/C abnormal group were higher than those in the A/C normal group, the differences were statistically significant (P〈0.05). And level of total cholesterol (TC) in the A/C abnormal group was lower than that in the A/C normal group, the differences were statistically significant (P〈0.05). Conclusion A/C ratio could predict level of glucose, lipid metabolism and cardiac function in patients with coronary heart disease complicated T2DM.
