摘要
目的:分析治疗前外周血血小板与绝对淋巴细胞计数比值(plateletlymphocyte ratio,PLR)与外周T细胞淋巴瘤非特指型(peripheral T cell lymphoma,unspecified,PTCL-U)患者临床病理特征的相关性,并探讨其在预后中的意义。方法:对天津医科大学肿瘤医院初诊的163例PTCL-U患者的临床资料及生存情况进行回顾性分析。根据受试者工作特征(receiver operating characteristic,ROC)曲线的截断值将患者分为低PLR组(PLR<170,80例)和高PLR组(PLR≥170,83例),分析治疗前PLR与PTCL-U患者预后的关系。结果:163例PTCL-U患者中,低PLR组患者的5年总生存(overall survival,OS)率和无进展生存(progression-free survival,PFS)率(76.3%和60.0%)均高于高PLR组(10.8%和8.4%,P值均<0.05)。年龄、美国东部肿瘤协作组体力状态(Eastern Cooperative Oncology Group performance status,ECOG PS)评分、B组症状、血清乳酸脱氢酶(lactate dehydrogenase,LDH)水平、血小板及PLR与PTCL-U患者的OS明显相关(P值均<0.01)。多因素分析结果显示,年龄、血清LDH水平、血小板及PLR是PTCL-U患者预后的独立影响因素(P值均<0.05)。结论:治疗前外周血PLR与PTCL-U患者的预后明显相关,高水平PLR提示PTCL-U患者的预后不良。
Objective： To identify the correlation between pre-treatment peripheral blood platelet-lymphocyte ratio （PLR） and the clinicopathological features of the patients with peripheral T cell lymphoma, unspecified （PTCL-U）, and to explore the prognostic significance of PLR in these patients.Methods： The clinical characteristics and survival state of 163 patients diagnosed of PTCL-U in Tianjin Medical University Cancer Institute and Hospital were analyzed retrospectively. All patients were divided into low PLR group （PLR 〈 170, n = 80） and high PLR group （PLR ~〉 170, n = 83） based on the receiver operating characteristic （ROC） curve＇s cut-off value. The relationship between pre-treatment PLR and the prognosis of these patients was analyzed. h PTCL-U, the 5-year overall sarvival （OS） and progression-free s in low PLR group （76.3% and 60.0%, respectively） were higher p （1 0.8% and 8.4%, respectively, both P 〈 0.05）. Age, Eastern performance status （ECOG PS）, B symptoms, serum lactate platelet and PLR were remarkably relevant to OS （all P 〈 0.01）. et and PLR were independent prognostic features for patients Conclusion： The peripheral blood PLR at pre-treatment is distinctly relevant to the prognosis of patients with PTCL-U. An elevated PLR predicts poor prognosis of these patients.
