摘要
目的观察胸腺基质淋巴细胞生成素(TSLP)在慢性阻塞性肺疾病急性加重(AECOPD)患者中的表达,并探讨其对AECOPD患者的临床意义。方法选取我院重症医学科2013年1月至2014年12月收治的41例AECOPD患者和41例非急性加重(COPD)患者。比较两组患者血清TSLP、降钙素原(PCT)及治疗前的白细胞介素-4(IL-4)、白细胞介素-5(IL-5)、白细胞介素-13(IL-13)、超敏C反应蛋白(hs-CRP)、白细胞计数(WBC)等指标,并作相关分析。结果 AECOPD组患者的TSLP水平为(48.49±19.63)p·ng-1·L-1,明显高于COPD组的(29.16±4.87)p·ng-1·L-1,差异有统计学意义(P<0.05);AECOPD组患者的IL-4水平为(8.42±2.46)ng/m L,略高于COPD组的(7.94±1.02)ng/m L,但差异无统计学意义(P>0.05);AECOPD组患者的IL-5水平为(14.93±2.01)ng/m L,明显高于COPD组的(4.93±1.42)ng/m L,差异有统计学意义(P<0.05);AECOPD组患者的IL-13水平为(402.16±24.62)ng/m L,明显高于COPD组的(109.83±12.55)ng/m L,差异有统计学意义(P<0.05);AECOPD组患者的PCT[(0.28±0.19)mg/L]、hs-CRP[(0.84±0.35)mg/L]、WBC[(7.49±1.80)×109]水平均低于COPD组,差异均具有统计学意义(P<0.05)。相关分析显示,AECOPD患者血清TSLP水平与患者的IL-4(r=0.114)、IL-5(r=0.052)、IL-13(r=0.101)水平呈正相关(P<0.05);血清TSLP水平与患者的PCT(r=-0.006)、hs-CRP(r=P0.110)、WBC(r=P0.130)水平呈负相关(P<0.05)。结论血清TSLP水平与AECOPD患者的病程发展有关;针对血清TSLP水平治疗急性加重慢性阻塞性肺疾病患者可能会有更显著的效果,血清TSLP水平检测可能在AECOPD患者的治疗中具有重要的价值。
Objective To observe the expression of thymic stromal lymphopoietin (TSLP) in patients withacute exacerbation of chronic obstructive pulmonary disease (AECOPD) and its clinical significance. Methods For-ty-one AECOPD patients (AECOPD group) and 41 patients with non-acute exacerbation of COPD (COPD group) wereselected from January 2013 to December 2014 in Critical Care Medicine of our hospital. Serum TSLP, procalcitonin(PCT), and interleukin-4 (IL-4), interleukin-5 (IL-5), interleukin-13 (IL-13), hypersensitive C-reactive protein(hs-CRP), white blood cell count (WBC) levels were compared and analyzed. Results TSLP level in AECOPD groupwas (48.49±19.63) p·ng-1·L-1, significantly higher than (29.16±4.87) p·ng-1·L-1 in COPD group, P〈0.05. IL-4 level inAECOPD group was (8.42±2.46) ng/mL, slightly higher than (7.94±1.02) ng/mL in COPD group, but the difference wasnot statistically significant (P〉0.05). IL-5 level in AECOPD group was (14.93±2.01) ng/mL, significantly higher than(4.93±1.42) ng/mL in COPD group, P〈0.05. IL-13 level in AECOPD group was (402.16±24.62) ng/mL, which was sig-nificantly higher than (109.83±12.55) ng/mL in COPD group (P〈0.05). The levels of PCT, hs-CRP and WBC in AECO-PD group, (0.28±0.19) mg/L, (0.84±0.35) mg/L, (7.49±1.80)×109 respectively, were significantly lower than those inCOPD group (P〈0.05). Correlation analysis showed that AECOPD patient’s TSLP levels had positive correlation withIL-4 (r=0.114), IL-5 (r=0.052), IL-13 (r=0.101) levels (P〈0.05), and were negatively correlated with PCT (r=-0.006),hs-CRP (r=-0.110), WBC (r=-0.130), P〈0.05. Conclusion TSLP levels in serum have an important relationship withthe disease progression of patients with AECOPD. Treatment based on serum TSLP levels for patients with AECOPDmay have a significant effect, and the detection of serum TSLP levels may have a broader application in AECOPD pa-tients.
