摘要
目的了解遵义地区女性HPV病毒感染的情况和特点,并研究HPV病毒感染与宫颈上皮细胞病变的关系。方法以2014年6月~2016年5月在遵义市第一人民医院就诊及体检的3 082例妇女为研究对象,采用实时荧光定量PCR进行HPV分型检测、同时对其中50例妇女进行宫颈组织病理学检查。结果 3 082病例标本中共得到阳性316例,其中HPV亚型58型、52型、16型检出率列前3位。2014年12月~2015年1月,2015年11月~2016年3月阳性数多于其他月份;对所有检验结果按不同年龄分为4组,<30岁组、30~40岁组,40~50岁组,>50岁组,各组间感染率无统计学差异;依CIN分级分组,HPV感染率在不同组间比较,BCC组与CINⅠ组,CINⅠ组与CINⅡ组,CINⅡ组与CINⅢ组,差异有统计学意义(P<0.05),CINⅢ组与SCC组无统计学差异。结论本地区女性HPV病毒感染率约为10%,主要以58型,52型,16型病毒为主;HPV病毒检出率随季节变化,秋冬季节阳性数较多;本地区各年龄层HPV易感性无显著性差异,均为HPV病毒重点筛查对象。HPV病毒阳性率与宫颈病变程度呈正相关,HPV病毒检测在宫颈病变筛查和防治过程中具有重要意义。
Objective The purpose of this study was to investigate the infection status of human papilloma virus in zunyi and explore the relationship of HPV infection and Cervical lesions. Methods 3 082 patients from gynecological outpatient department in the first people＇s Hospital of Zunyi from June 2014 to May 2016 were detect infection status of HPV by RT-PCR,50 patients of them were detected by colposcope and biopsy.Results The infection ones of the 3 082 patients were 316,the top three HPV genotype distribution positive rate were HPV58, HPV52 and HPV16.There were more infected ones from December 2014 to January 2015, November 2015 to March 2016. There wasn＇t statistically significant difference between 30 year old group, 31 to 40 year old group,41 to 50 year old group,and 70 year old group.But there was statistically significant difference between BCC group and CIN Ⅰgroup, CIN Ⅰgroup and CIN Ⅱgroup, CIN Ⅱgroup and CIN Ⅲgroup,which were grouped by the Pathologic grading. Conclusion The infection rate of HPV in Zunyi was about 10%. The top three HPV genotype distribution were HPV58,HPV52 and HPV16. HPV infection rate is related to the season. There were more infected ones in the autumn and winter. There were no significant difference for the age of the infection.So,there is necessity to carry out HPV detection for all age group.The infection rate increased with the severity of cervical lesions,so the important role of HPV detection have been proved.
