摘要
目的观察系统排便训练对脑卒中患者便秘的预防价值。方法选取2013年5月至2016年5月本院收治的78例脑卒中住院患者为研究对象,随机分为干预组与常规组,每组各39例。常规组采取卒中后常规护理,干预组在常规护理基础上进行系统性排便训练。比较两组便秘发生率、干预前后Wexner便秘评分与直肠肛管压力。结果干预组便秘发生率为10.23%,显著低于常规组(30.77%),差异有统计学意义(P<0.05)。干预组干预1周、2周、4周后的Wexner便秘评分分别为(13.05±2.25)分、(11.42±2.00)分、(9.61±1.34)分,均显著低于常规组同一时点,差异均有统计学意义(P<0.05)。干预组干预4周后的初始感觉阈值、初始排便阈值、最大耐受阈值分别为(27.49±3.74)m L、(75.61±6.98)m L、(114.25±12.22)m L,均低于本组干预前与常规组干预后同一时点,差异均有统计学意义(P<0.05)。结论脑卒中患者早期进行系统排便训练可降低患者Wexner便秘评分与直肠肛管压力,促进排便,利于减少便秘发生。
Objective To study the preventive effects of defecation training on constipation in stroke patients.Methods 78 patients with stroke were randomly divided into intervention group and routine treatment group,with 39 cases in each group.The routine treatment group received routine nursing after stroke and the intervention group received systematic defecation training along with routine nursing.The incidence of constipation,the Wexner constipation score and anorectal pressure were compared between the two groups.Results The incidence of constipation was significantly different between intervention group and routine group[10.23% vs 30.77%]（P〈0.05）.The Wexner constipation score of the intervention group at 1weeks,2 weeks,4 weeks [（13.05±2.25） points,（11.42±2.00）points,（9.61±1.34） points,respectively] were significantly lower than those of the routine treatment group（P〈0.05）.At 4 weeks,the initial sensory threshold,initial defecation threshold and maximum tolerated threshold of intervention group [（27.49±3.74） mL,（75.61±6.98） mL,（114.25±12.22） mL,respectively] were lower than those of the intervention group（P〈0.05）.Conclusion Early intervention with systematic defecation training in stroke patients can reduce the Wexner constipation score and anorectal pressure,promote defecation and prevent constipation.
