摘要
目的探讨D-乳酸跨肺梯度联合P-选择素和血小板参数检测在肺炎疗效评估中的作用。方法收集115例肺炎患者,分为重症肺炎组(50例)和轻症肺炎组(65例),另选60例健康体检者为对照组,检测并比较3组间及重症肺炎患者治疗前后PLT、PDW、MPV、P-LCR的差异。结果重症肺炎组D-乳酸跨肺梯度、P-选择素、PLT均显著高于轻症肺炎组和对照组;PDW、MPV、P-LCR均显著低于轻症肺炎组和对照组;轻症肺炎组D-乳酸跨肺梯度、PLT均显著高于对照组,上述差异均有统计学意义(P<0.05)。与治疗前比,重症肺炎患者治疗后第3 d D-乳酸跨肺梯度显著降低,其余指标治疗后第5 d变化差异有统计学意义(P<0.05)。结论 D-乳酸跨肺梯度联合P-选择素和血小板参数检测可更快、更准确地评价肺炎病情、疗效和判断预后。
Objective To explore the detection value of D-lactate gradient across the lung,P-selection and platelet parameter in the diagnosis and therapeutic evaluation of pneumonia.Methods 115 patients with pneumonia were selected in our hospital from January 2013 to January 2015,and they were divided into the severe pneumonia group(50 cases) and mild pneumonia group(65 cases) according to the illness severity.60 healthy individuals were selected as the control group.The levels of D-lactate gradient across the lung,P-selection and platelet parameter(PLT,PDW,MPV and P-LCR) of research objects among the 3 groups were detected and compared,as well as patients in the severe pneumonia group before and after treatment.Results Compared with the mild pneumonia group and control group,the levels of D-lactate gradient across the lung,P-selection and PLT in the severe pneumonia group were significantly higher,but the levels of PDW,MPV and P-LCR were greatly lower.Compared with the control group,the level of D-lactate gradient across the lung and PLT in the mild pneumonia group were significantly higher,with the differences statistically significant(P〈0.05).The level of D-lactate gradient across the lung in the severe pneumonia group after 3 days of treatment greatly decreased than before,and the other observation targets had significant difference after 5 days of treatment,with the differences statistically signficant(P〈0.05).Conclusion The simultaneous determination of D-lactate gradient across the lung,P-selection and platelet parameter may be faster and more accurate in therapeutic evaluation and prognosis for pneumonia.
