摘要
目的探讨促性腺激素释放激素激动剂(GnRH-a)联合腹腔镜手术治疗子宫内膜异位症的近远期疗效。方法选取该院2012年12月-2014年2月收治的50例仅接受腹腔镜治疗的子宫内膜异位症患者作为对照组,另选取同期收治的55例接受腹腔镜联合GnRH-a治疗的患者作为观察组,对比两组患者的临床疗效、血清学指标变化及随访2年妊娠率、生育率及流产率。结果观察组总有效率明显高于对照组,差异有统计学意义(χ~2=6.45,P<0.05)。两组患者治疗后4周、治疗后12周较治疗前相比,CA125、CA19-9、PP14、HE4水平均明显降低;观察组较对照组相比,治疗后4周、治疗后12周的CA125、CA19-9、PP14、HE4水平下降更加明显,差异有统计学意义(均P<0.05)。观察组较对照组相比妊娠率、生育率均较高,复发率较低,差异有统计学意义(均P<0.05)。结论在腹腔镜治疗子宫内膜异位症基础上加用GnRH-α可有效改善患者的临床症状及体征,改善血清学指标,提高妊娠率及生育率,近远期疗效显著。
Objective To explore the short- term and long- term curative effect evaluation of gonadotropin releasing hormone agonist（ GnRH-a） combined with laparoscopic surgery in treatment of endometriosis. Methods Fifty patients with endometriosis who were treated by laparoscopic surgery in the hospital from December 2012 to February 2014 were selected as control group,55 patients with endometriosis who were treated by laparoscopic surgery and GnRH- a during the same period were selected as observation group. The clinical curative effects,changes of serological indicators,two-year pregnancy rates,two-year fertility rates and two-year abortion rates in the two groups were compared. Results The total effective rate in observation group was statistically significantly higher than that in control group（ χ2=6. 45,P〈0. 05）. The levels of CA125,CA19- 9,PP14,and HE4 after 4 and 12 weeks in the two groups were significantly lower than those before treatment. The levels of CA125,CA19-9,PP14,and HE4 after 4 and 12 weeks in observation group were statistically significantly lower than those in control group（ P〈0. 05）. Compared with control group,the pregnancy rate and fertility rate in observation group were higher,the recurrence rate was lower,and the difference was statistically significant（ P 〈0. 05）. Conclusion GnRH-a combined with laparoscopic surgery can effectively improve clinical symptoms and signs,serological indicators,pregnancy rate and fertility rate of patients with endometriosis,the short-term and long-term curative effect are significant.
