摘要
目的探讨H-钙黏蛋白表达与结肠癌预后的关系。
方法免疫组织化学检测结肠癌临床样本中H-钙黏蛋白的表达，χ2检验分析计数资料的差异；Kaplan-Meier法绘制生存曲线，Log-rank法对生存分析进行检验；Cox比例风险回归模型分析患者预后的独立影响因素。
结果与癌旁正常组织比较，H-钙黏蛋白在癌组织中表达均降低；H-钙黏蛋白的表达与患者性别、年龄、肿瘤大小、位置等因素之间无明显相关，与肿瘤TNM分期（P＝0.003），肿瘤分化程度（P＝0.000），淋巴转移（P＝0.013）显著相关。Cox分析显示肿瘤TNM分期[风险比（HR）＝3.624，95%可信区间（CI）：1.875～9.227，P＝0.009]、肿瘤分化程度（HR：1.677，95%CI：0.855～3.964，P＝0.031）、淋巴转移（HR：6.680，95%CI：3.336～12.028，P＝0.003）和H-钙黏蛋白表达（HR：2.927，95%CI：0.998～8.499，P＝0.012）是结肠癌患者生存的独立影响因素。Kaplan-Meier生存分析表明，与H-钙黏蛋白表达水平相对较高的患者比较，H-钙黏蛋白表达相对较低的患者预后生存较差（P＝0.000）。
结论H-钙黏蛋白是结肠癌患者预后的独立影响因素。
Objective To investigate relationship of H -cadherin expression with the prognosis of colorectal cancer. Methods Expression of H - cadherin in colon cancer tissues and adjacent normal colon tissues were detected by immunohistochemistry （IHC）. Difference of variables was analyzed by Chi square test. Survival curve was plotted by Kaplan - Meier method with Log - rank test. Cox proportional hazard re- gression model was used to evaluate the independent prognostic factors. Results Expression of H - cadherin was decreased in colon cancer tissues compared with adjacent normal tissues. H - cadherin expression level in cancer tissues did not correlate with gender, age, tumor size and site. H -cadherin ex- pression statistically significantly correlated with TNM stage （ P = 0. 003 ）, tumor differentiation （ P = 0. 000） and lymph nodal metastasis （P = 0. 013）. Cox regression analysis indicated that TNM stage [ haz- ard ratio （HR） = 3. 624, 95% confidence interval （CI） ： 1. 875 - 9. 227, P = 0. 009 ], tumor differentia- tion （HR： 1. 677, 95% CI： 0. 855 - 3. 964, P =0. 031 ）, lymph nodal metastasis （FIR： 6. 680, 95% CI： 3. 336 - 12. 028, P =0. 003） and H - cadherin expression （HR： 2. 927, 95% CI： 0. 998 - 8. 499, P = 0. 012） were the independent prognostic factors of colon cancer patients. Kaplan- Meier survival analysis showed that, compared with patients with relative higher H - eadherin expression, patients with relative lower H - cadherin expression had poorer overall survival （ P = 0. 000）. Conclusion H - cadherin is an independent prognostic factor of colon cancer patients.
