毒性甲状腺肿和桥本甲状腺炎组织剪切波速度的临床研究 被引量:4

摘要 目的探讨毒性甲状腺肿又称Graves病(Graves disease,GD)初发患者和桥本甲状腺炎(Hashimoto's thyroiditis,HT)甲状腺功能亢进(甲亢)患者各项参数与甲状腺组织剪切波速度(shear wave velocity,SWV)的相关性。方法确诊为初发GD的患者45例,HT甲亢患者37例,健康志愿者50例作为对照组。3组均取静脉血检测甲状腺功能,由同一资深医生完成甲状腺二维超声扫查记录及SWV测量,比较GD组、HT甲亢组及对照组的SWV以及与促甲状腺激素(thyrotropin,thyroid stimulating hormone,TSH)、游离三碘甲腺原氨酸(free tri-iodothyronine,FT3)、游离甲状腺素(free thyroxine,FT4)的相关性。结果 GD组甲状腺上动脉流速高于HT甲亢组,差异有统计学意义(P<0.05),2组间血流分级差异无统计学意义(P>0.05);GD组体积明显大于HT甲亢组和对照组,差异有统计学意义(P<0.05);GD组和HT甲亢组SWV值均高于对照组(P<0.05)。GD和HT甲亢组的TSH、FT3、FT4与甲状腺组织的SWV值无相关性(P>0.05)。结论剪切波弹性成像虽然能够客观定量评估甲状腺组织的弹性,但对GD和HT甲亢患者鉴别诊断较为困难,GD患者体积显著增大和甲状腺上动脉流速明显增高对两者的鉴别有一定价值。 Objective To investigate the changes of the shear wave velocities (SWV of thyroid tissues among patients with Graves disease(GD) and Hashimoto's thyroiditis(HT) by using Shear Wave velocity. Methods Forty-five cases who were diagnosed with initial GD and 37 cases who were diagnosed HT and 50 healthy volunteers as control group were included in the study. The fasting blood samples were taken from every case in the morning, the thyroid function and related antibody were detected. SWV of thyroid tissues was measured by the same doctor. SWV was compared between GD group, HT group and control group. The correlation between thyrotropin, thyroid stimulating hormone (TSH), free tri-iodothyronine (FT3), free thyroxine (FT4) levels and SWV was analyzed. Results Thyroid artery flow velocity in GD group is higher than the HT group, the difference was statistically significant ( P〈 0. 05 ). But there was no statistical differende in color flow grade(P〈0.05). Thyroid volumes of cases in GD group were significantly higher than those of cases in HT and control group(P〈0.05). SWV values in GD and HT group are higher than the control group(P〈0.05). No correlation was detected between thyroid function and SWV value in GD and HT group(P〉0.05). Conclusion SWE imaging can be objective and quantitative assessment thyroid tissue, but it is difficult to distinguish patients with GD and HT. Marked increased volume and the flow in upper thyroid artery both play a certain value in differential GD and HT.
