摘要
目的探讨声脉冲辐射力成像技术(ARFI)诊断甲状腺弥漫性疾病的价值。方法选取2012年8月—2013年8月在我院内分泌科和腺体外科确诊为甲状腺弥漫性疾病的患者106例为病例组,其中桥本甲状腺炎50例,Graves'病56例。另选择同时期我院体检中心健康志愿者(甲状腺各项指标正常且无甲状腺相关疾病病史)128例为对照组。用线阵探头、凸阵探头分别测量6次甲状腺的硬度,以横向剪切波速度(SWV,m/s)表示;比较甲状腺弥漫性疾病和正常甲状腺的SWV值。结果分别用线阵探头与凸阵探头测量病例组、对照组甲状腺SWV值,病例组均高于对照组(P<0.05);两种探头之间测量的甲状腺SWV值比较,差异均无统计学意义(P>0.05)。桥本甲状腺炎组甲状腺SWV值均高于Graves'病(P<0.05)。以SWV=2.1 m/s为临界点,受试者工作特征曲线下面积为0.81,诊断甲状腺弥漫性疾病的灵敏度、特异度、阳性预测值、阴性预测值、准确率分别为72.6%、81.3%、86.5%、64.2%、75.9%。结论 ARFI对鉴别甲状腺弥漫性疾病和正常甲状腺有一定价值。
Objective To investigate the value of acoustic radiation force impulse( ARFI) elastography in diagnosis of diffuse diseases of thyroid gland. Methods 106 patients( 56 cases of Graves' disease,50 cases of Hashimoto thyroiditis) who were diagnosed with diffuse diseases of thyroid gland in department of endocrinology and gland surgery of our hospital from August2012 to August 2013,were selected as disease group. At the same time,128 healthy volunteers( each index of thyroid gland was normal,no history of thyroid- associated disease) were selected as control group. The stiffness of thyroid was detected for 6 times using a convex probe and a linear probe,respectively,the stiffness was expressed by shear wave velocity( SWV,m /s). The SWV values of thyroid were compared between disease group and control group. Results The SWV values of thyroid among patients in disease group were significantly higher than those among patients in control group( P 〈0. 05). The SWV values indicated that there was no remarkable difference between convex probe method and linear probe method( P 〉0. 05). The SWV values of thyroid among patients with Hashimoto thyroiditis were significantly higher than those among patients with Graves' disease( P 〈0. 05). SWV = 2. 1 m /s was set as critical point,the area under the receiver operating characteristic( AUR) curves was 0. 81,the sensitivity,speciality,predictive positive rate,predictive negative rate and accuracy in diagnosis of diffuse diseases of thyroid gland were 72. 6%,81. 3%,86. 5%,64. 2% and 75. 9%,respectively. Conclusion ARFI has important value in differential diagnosis of diffuse diseases of thyroid gland from normal thyroid.
