烧伤病人血液炎性细胞因子水平变化的研究 被引量:12

摘要 目的探讨烧伤病人血液炎性细胞因子水平变化。方法选取2014年1月-2015年6月期间我院收治的55例烧伤患者作为研究对象,设为观察组,随机选取同期我院体检的健康人群30例作为对照组,比较两组患者血液炎性因子表达水平,烧伤患者烧伤后1天和第3天血液炎性细胞因子水平以及生存组和死亡组烧伤后1天和第3天血液炎性细胞因子水平。结果烧伤患者组血清G-CSF、GM-CSF、IL-1RA、IL-15、MCP-1、MIP-1β以及VEGF浓度显著高于健康对照组(P<0.05),烧伤患者组血清IL-8和IL-17A浓度显著低于健康对照组(P<0.05);烧伤患者第3天血清G-CSF和IL-6浓度显著高于烧伤患者第1天(P<0.05),烧伤患者第3天血清GM-CSF、IFN-γ、IL-1RA、IL-2、IL-9、IL-10、IL-17A、MIP-1β、PDGF-BB、RANTES、VEGF以及Eotaxin浓度显著低于烧伤患者第1天(P<0.05);死亡组患者烧伤后第1天血清IL-1RA、IL-6以及MCP-1浓度显著高于生存组患者(P<0.05),死亡组患者烧伤后第3天血清PDGF-BB浓度显著低于生存组患者(P<0.05)。结论炎性细胞因子相关通路相互作用形成复杂网络参与烧伤患者的血液炎性细胞因子水平调控,且IL-1RA、IL-6、MCP-1和PDGF-BB可作为烧伤患者预后指标,值得临床治疗过程中加以关注。 Objective To investigate the changes of inflammatory cytokines and their prognostic ability in cases of major burn injury.Methods During January 2014 to June 2015 our hospital,55 cases of burn patients admitted to the observation group,and randomly selected30 cases of healthy people of hospital examination as a control group in the same period,to compare blood inflammatory cytokine expression levels of the two groups,and blood levels of inflammatory cytokines of burn patients with burns after a day and three days and blood levels of inflammatory cytokines of a day and three days after burn of survival group and death group.Results serum G-CSF,GM-CSF,IL-1RA,IL-15,MCP-1,MIP-1βand VEGF of burn patients were significantly higher than the control group(P〈0.05),and serum IL-burn patients 8and IL-17 A was significantly lower than the control group(P〈0.05);serum G-CSF and IL-6concentration of three days after burn was significantly higher than that on day 1(P〈0.05),and serum GM-CSF,IFN-γ,IL-1RA,IL-2,IL-9,IL-10,IL-17 A,MIP-1,PDGF-BB,RANTES,VEGF and Eotaxin concentrations f three days after burn were significantly lower than those on 1day(P〈0.05);serum IL-1RA,IL-6,and MCP-1concentrations of the death group after 1day were significantly higher than those of the survival of patients(P〈0.05),and three days after burn,serum PDGF-BB of the death group was significantly lower than that of the survival group(P〈0.05).Conclusion The inflammatory cytokines related pathways interact and form complex networks to regulate blood levels of inflammatory cytokines involved of burn patients,and IL-1RA,IL-6,MCP-1and PDGF-BB can be used as a prognostic indicator of patients with burns,it was worthy of to be concerned about the course of treatment.
