Application of CBL+Mini-CEX in the Clinical Ability Culture of Hoase Physicians

摘要 目的探讨CBL法结合Mini-CEX应用于住院医师规范化培训过程中的应用效果。方法将2016年5月—2016年10月在齐齐哈尔医学院附属第三医院进行住院医师规范化培训的24名住院医师按照不同教学方式分组,分为CBL+Mini-CEX组(12名)及对照组(12名),CBL+Mini-CEX组采用Mini-CEX联合CBL法进行教学,对照组采用传统带教法进行教学。2组完成临床学习后进行理论考试及Mini-CEX测评。对两组住院医师的理论成绩及Mini-CEX测评分数进行比较。结果 CBL+Mini-CEX组理论成绩为(86.68±8.01),对照组理论成绩为(80.35±6.52),两组理论成绩之间的差异具有统计学意义(P<0.05),CBL+Mini-CEX组高于对照组。CBL+Mini-CEX组在医疗面谈、体格检查、临床判断、人文关怀、沟通技能、组织效能、整体表现的成绩分别为(6.57±1.08)、(7.21±1.15)、(7.37±1.12)、(6.92±0.87)、(7.18±1.21)、(7.66±1.03)、(7.68±0.87),对照组在医疗面谈、体格检查、临床判断、人文关怀、沟通技能、组织效能、整体表现的成绩分别为(5.65±0.87)、(5.59±0.97)、(6.38±0.99)、(6.15±0.68)、(6.21±0.87)、(6.51±0.93)、(6.32±1.08),两组Mini-CEX测评7个方面之间的差异均有统计学意义(P<0.05),CBL+Mini-CEX组均高于对照组。结论 CBL联合Mini-CEX法能够提高住院医师的临床能力。 Objective To study the application effect of CBL +Mini-CEX in the standardized training course of hoase physicians. Methods 24 hoase physicians with standardized training in the Third Affiliated Hospital of Qiqihar Medical School from May 2016 to October 2016 were divided into two groups according to different teaching methods with 12 cases in each, the CBL+Mini-CEX group adopted the Mini-CEX and CBL for teaching, while the control group adopted the traditional teaching method, and the theoretical examination and Mini-CEX evaluation of the two groups were carried out after clinical study, and the theoretical score and Mini-CEX evaluation score of the two groups were compared. Results The difference in the theoretical score between the CBL+Mini-CEX group and the control group had statistical significance(P〈0.05), [(86.68±8.01) vs(80.35±6.52)], and the scores in the medical interview, physical examination, clinical judgment, humanistic care, communication skill, organizational efficacy and holistic performance in the CBL+Mini-CEX group were higher than those in the control group with statistical significance(P〈0.05), [(6.57±1.08),(7.21 ±1.15),(7.37 ±1.12),(6.92 ±0.87),(7.18±1.21),(7.66±1.03),(7.68±0.87) vs(5.65±0.87),(5.59±0.97),(6.38±0.99),(6.15±0.68),(6.21±0.87),(6.51±0.93),(6.32±1.08)]. Conclusion The CBL+Mini-CEX method can improve the clinical ability of hoase physicians.
