摘要
目的探析腹部B超与阴道B超在异位妊娠上的临床诊断价值。方法选取广州市花都区妇幼保健院胡忠医院与花都人爱医院于2013年3月—2016年7月收治的900例异位妊娠患者为此次研究对象,所有患者均经病理活检确诊为异位妊娠。根据数字随机表达法将900例患者平均分为对照组和观察组,每组各450例。对照组患者接受腹部B超进行检查和诊断,观察组患者则接受阴道B超进行检查诊断,对两组患者的临床诊断效果进行随机对照和对比分析。结果在诊断符合率上,观察组患者的临床诊断符合率为95.11%,对照组为86.22%,两组数据相比差异有统计学意义(P<0.05);在异位妊娠分型诊断上,观察组的破裂型异位妊娠检出率明显高于对照组(P<0.05),但两组在流产型和未破裂型异位妊娠的检出率上差异无统计学意义(P>0.05);在B超图像检查结果上,观察组的假孕囊、胚芽以及附件包块检出率均高于对照组(P<0.05),但两组在盆腔积液、原始心管搏动检出率上差异无统计学意义(P>0.05)。结论在异位妊娠的临床诊断上,相对于腹部B超,阴道B超的诊断符合率、灵敏度更高。同时,阴道B超在异位妊娠临床诊断上具有应用简单、安全性高、无痛等优势,可获得较好的临床诊断效果,可在临床上进一步应用和推广。
Objective To study the diagnostic value of abdominal ultrasonography and transvaginal ultrasonography in ectopic pregnancy. Methods 900 cases of ectopic pregnancy treated in huadu district maternal and child health hospital huzhong hospital and huadu ren＇ai hospital from March 2013 to July 2016 were selected as subjects of this study. All patients were diagnosed by pathological biopsy. Bit of pregnancy. According to the digital random expression method,900 patients were divided into control group and observation group,450 cases in each group. The patients in the control group were examined and diagnosed by abdominal ultrasonography. The patients in the observation group were diagnosed by vaginal ultrasonography,and the clinical diagnosis of the two groups were compared and analyzed.Results The diagnostic accuracy was 95. 11% in the observation group and 86. 22% in the control group. The difference was statistically significant（P〈0. 05）. In the diagnosis of ectopic pregnancy,The detection rate of ruptured ectopic pregnancy in the observation group was significantly higher than that in the control group（P〈0. 05）,but there was no significant difference between the two groups in the detection rate of the ectopic pregnancy and the unruptured ectopic pregnancy（P〈0. 05）. But in the pelvic effusion,the detection rate of the primordial heart tube pulsation was significantly higher than that in the control group（P〈0. 05）,and the detection rate of false pregnancy sac,The difference was not statistically significant（P〈0. 05）. Conclusion In the clinical diagnosis of ectopic pregnancy,compared with abdominal B-ultrasound,vaginal B-ultrasound diagnostic accuracy,sensitivity is higher. At the same time,vaginal B-ultrasound in the clinical diagnosis of ectopic pregnancy has a simple application,high security,painless and other advantages,can obtain better clinical diagnostic results,can be further applied in clinical and promotion.
