摘要
目的探讨血必净对爆炸致家兔急性肺损伤(ALI)的保护作用及其机制。方法健康清洁级家兔32只,随机分成正常对照组、ALI组和低剂量血必净组、高剂量血必净组,每组8只。爆炸致伤家兔,建立ALI动物模型后,低剂量血必净组、高剂量血必净组分别注射10、20 ml/kg血必净溶液,正常对照组、ALI组注射生理盐水。4 h采集血液,24 h采集支气管肺泡灌洗液(BALF)和肺组织标本,Western blot法和反转录PCR法测定肺组织MMP-9蛋白和mRNA表达水平,ELISA法测定BALF及血清中肿瘤坏死因子(TNF)-α、白介素(IL)-6、IL-10的含量,并测定肺湿干重比(W/D),光镜下观察肺组织病理学变化。结果 ALI组MMP-9、TNF-α、IL-6、IL-10表达量以及W/D均较正常对照组升高(P<0.05)。经血必净治疗后,低剂量血必净组、高剂量血必净组的MMP-9、TNF-α、IL-6表达量以及W/D均较ALI组降低(P<0.05),IL-10表达量较ALI组升高(P<0.05),均以高剂量组改变最显著(P<0.05)。肺病理切片显示ALI组肺泡及间质水肿,肺泡腔内有浆液性渗出物及大量炎性细胞、红细胞渗出,低剂量血必净组、高剂量血必净组肺损伤程度明显减轻。结论血必净可通过降低MMP-9的表达抑制失控性炎症反应,减轻肺损伤,保护肺组织。
Objective To investigate the protective effect and mechanism of Xuebijing on acute lung injury(ALI) in rabbits induced by blast. Methods A total of 32 healthy and clean rabbits were randomly divided into 4 groups : normal control group, ALI group, low-dose Xuebijing group and high-dose Xuebijing group, and each group has 8 rabbits. After the establishment of ALI animal model of rabbits induced by blast, the rabbits in low-dose Xuebijing group and high-dose Xuehijing group were intravenously injected with different doses of Xuebijing( 10,20 ml/kg) , while the rabbits in normal control group and ALI group were injected with normal saline. The blood was saved after 4 hours, and the bronchial alveolar lavage fluid (BALF) and lung tissue were collected at 24 hours, the wet to dry weight ratios(W/D) were determined, and the levels of tumor necrosis factor-α (TNF-α) , intedeukin( IL-6, IL-10 ) in the BALF andserum were measured by ELISA methods ; the expression level of MMP-9 protein and mRNA in the lung tissue was detected by Western blot and reverse transcription-polymerase chain reaction; the pathological changes of lung tissues were observed under the light microscope. Results The levels of W/D, TNF-α, IL-6, IL-IO, MMP-9 in ALI group were markedly higher than those in normal control group ( P 〈 0.05 ).
