摘要
目的探讨应用超微血流成像(SMI)技术诊断颈动脉狭窄及斑块内新生血管的临床价值。方法选择颈动脉硬化斑块患者30例,44个斑块,应用SMI评价颈动脉狭窄程度及对斑块内新生血管探查并分级,评价SMI与CT血管造影(CTA)及超声造影(CEUS)结果的一致性。结果 SMI评价颈动脉狭窄程度与CTA有良好的一致性(Kappa=0.757,P<0.001);44个斑块中,检测出新生血管(SMI42个,CEUS41个),SMI评价斑块内新生血管分级与CEUS有较好的一致性(Kappa=0.674,P<0.001)。结论 SMI可无创的评价管腔狭窄程度及检测斑块内的新生血管。
Objective To investigate the clinical value of superb micro-vascular imaging (SMI) on the diagnosis of the degree of carotid artery stenosis and the neovascularization in carotid artery. Methods The carotid arteries of 30 cases suffering from atheroscierotic plaques were examined, including 44 plaques. We used SMI to evaluate the degree of carotid artery stenosis and the grading of the neovascularization in carotid plaque. The consistency of the results of SMI and computer tomography angiography (CTA) as well as contrast-enhanced ultrasound (CEUS) were evaluated. Results SMI had a fairly well consistency to evaluate the grade of neovascularization within atherosclerotic compared with CTA( Kappa = 0. 757, P 〈 0. 001 ) ; In 44 plaques, 42 neo- vascularizations were found by SMI and 41 were detected by CEUS. SMI exhibited a satisfying consistency with CEUS of evaluating the grade of neovascularization within atherosclerotie( Kappa = 0. 674, P 〈 0. 001 ). Conclusion SMI can evaluate the degree of carotid arterystenosis and detect the neovascularization in carotid plaques.
