踝肱指数在2型糖尿病患者下肢动脉病变中临床意义 被引量:3

摘要 目的探讨踝肱指数(ABI)在2型糖尿病患者下肢动脉病变中临床意义。方法随机抽取90例2型糖尿病患者,检测ABI值,根据ABI值分为糖尿病外周血管病(PAD)组(n=32)和非PAD组(n=58),分别检测两组患者的体重指数(BMI)、血压、空腹血糖、餐后2h血糖(2hPG)、糖化血红蛋白(HbAIC)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL—C)、高密度脂蛋白(HDL—C)、血尿酸(BUA)、尿蛋白(PRO),进行组间比较。结果PAD组与非PAD组比较,BMI、病程、收缩压(SP)、空腹血糖、2hPG、HbAIC、TC、LDL-C、BUA、PRO差异有统计学意义(P〈0.05),舒张压(DP)、甘油三脂、HDL—C差异无统计学意义(P〉0.05)。结论血压、血糖、血脂、BUA紊乱及PR0含量升高促发糖尿病下肢动脉病变,ABI检测在早期诊断PAD疾病中起到较好的预防作用,值得在临床中推广。 Objective To investigate the value of clinical application of ankle brachial index in the lower extremity artery disease ( LEAD ) . Methods 90 cases of type 2 diabetic were randomly divided into two groups of nonperipheral artery disease ( NPAD ) group and peripheral artery disease ( PAD ) group according to ankle brachial index. Their clinical data were analyzed, including body mass index ( BMI ) , systolic blood pressure ( SBP ) , diastolic blood pressure ( DBP ) , fasting plasma glucose ( FPG ) , 2hours postprandial glucose ( 2hPG ) , glycosylated hemoglobin ( HbAIC ) , triglycerides ( TG ) , total cholesterol ( TC ) , high density lipoprotein cholesterol ( ( HDL-C ) , low density lipoprotein cholesterol ( LDL-C ) , blood uric acid ( BUA ) , 24 hours urinary albumin. These results were compared and analyzed. Results Compared with non-PAD group, disease duration, body mass index, systolic blood pressure, fasting plasma glucose (FPG) , 2hours postprandial glucose, glycosylated hemoglobin, total cholesterol, low density lipoprotein cholesterol, 24 hours urinary albumin and blood uric acid of PAD group were significant increased ( P〈0.05 ) . There was no significance in diastolic blood pressure, triglycerides and high density lipoprotein cholesterol (P〉0.05 ) . Conclusions The disorder of blood pressure, blood glucose, blood lipid and blood uric acid with abnormal proteinuria has promotion effect in occurrence of peripheral artery disease. The examination of ABI has good prevention effect in early diagnose of peripheral artery disease, and it is worth clinical application.
